An 18-year-old woman presented with a pruritic eczematous eruption that developed after topically applying an ointment containing hydrocortisone acetate, neomycin sulfate, and Centella asiatica (426). She was positive to all three ingredients of the ointment. 

In 145 patients with eczema of the external ear canal, allergic contact dermatitis was diagnosed in one-third topical therapeutic agents, especially neomycin sulfate and probably polymyxin B, were the dominating allergens (22).

Neomycin sulfate is available for topical and oral administration. Neomycin and polymyxin B have been used for irrigation of the bladder. For this purpose, 1 mL of a preparation (neosporin G. u. irrigant) containing 40 mg neomycin and 200,000 units polymyxin B per milliliter is diluted in 1 L of 0.9% NaCl solution and is used for continuous irrigation of the urinary bladder through appropriate catheter systems. The goal is to prevent bacteriuria and bacteremia associated with the use of indwelling catheters. 

Gentamycin has been advocated for topical treatment because it was a non-sensitizer in prophetic patch tests, (Black et al. 1963). Bandmann and Mutzeck (1973) examined 600 contact dermatitis patients and found 5% positive to neomycin sulfate, while negative to gentamycin, 0.8% positive to both, and 0.3% negative to neomycin sulfate while positive to gentamycin. 

Commercial preparation of this topi-cal antibiotic contains 9-22 bacitracin analogs, of which Bacitracin A is the most active used only in topical applications due to nephrotoxicity typically used in creams With polymyxin B, or polymyxin B and neomycin sulfate produced by BturiUus lichenifomus.
