Surgical implantation of 90Y has been used in almost all radio therapeutic treatment of human tumors. The method has been successfully used in the treatment of Nelson s syndrome [177], Cushing s syndrome [178] and breast cancer [179]. 

Bilateral adrenalectomy surgery had been the mainstay of therapy for years. It is used now only in patients for whom transsphenoidal surgery and pituitary irradiation have failed or caimot be used. Bilateral adrenalectomy rapidly reverses hypercortisolism. However, patients may develop Nelson s syndrome, which involves seUa turcica enlargement and hyperpigmentation, caused by postoperative hypothalamic stimulation. Therefore if bilateral adrenalectomy is used it should be accompanied by some form of hypothalamic inhibition, such as cyproheptadine. 

Increases are found in pituitary-dependent Cushing s syndrome. Nelson s syndrome, Addison s disease, congenital adrenal hyperplasia and ectopic ACTH production from a tumour.

Corticotropin secretion is unaff ed by GH-RIH in normal subjects (H3) both basally and in response to insulin hypoglycemia. However, Tyrrell et al. (Tl) have shown a significant faJl in elevated ACTH levels in Nelsons syndrome (4 patients) and in Cushing s disease (1 patient) in response to GH-RIH infusions. These results suggest that the mechanism determining release of ACTH by adenoma cells differ from that in normal pituitary cells.
