Nausea, during pregnancy

Toxicological studies have demonstrated that there are no important problems with fluconazole. Therapeutic doses of fluconazole may cause enzyme induction in the Hver. This suggests that interactions with other dmgs cannot be excluded. The side effects are similar to those of itraconazole and include nausea, headache, and vertigo. Occasionally, increased Hver enzymes may be noted. Like itraconazole, fluconazole is contraindicated during pregnancy. 

Recommend nonpharmacologic therapy for the following disorders when they occur during pregnancy or lactation nausea and vomiting, constipation, heartburn, pain, and cough and cold. 

Hyperemesis gravidarum A rare disorder of severe and persistent nausea and vomiting during pregnancy that can result in dehydration, malnutrition, weight loss, and hospitalization. 

Metoclopramide may have some potential value (10 mg orally or IV 30 minutes before each meal and at bedtime) in nausea and vomiting of a variety of etiologies (uncontrolled studies report 80% to 90% efficacy), including emesis during pregnancy and labor (5 to 10 mg orally or 5 to 20 mg IV or IM, 3 times a day). 

Hepatotoxicity is the major concern in 15% of pyrazinamide recipients. It also can inhibit excretion of urates, resulting in hyperuricemia. Nearly all patients taking pyrazinamide develop hyperuricemia and possibly acute gouty arthritis. Other adverse effects include nausea, vomiting, anorexia, drug fever, and malaise. Pyrazinamide is not recommended for use during pregnancy.

Roman chamomile is stated to have carminative, antiemetic, antispasmodic, and sedative properties. It has been used for dyspepsia, nausea and vomiting, anorexia, vomiting during pregnancy, dysmenorrhea, and specifically for flatulent dyspepsia associated with mental stress. 

Toxic symptoms associated with excessively large applications include nausea, vomiting, alterations in sensorium, muscle weakness, neuropathy with diminished tendon reflexes, coma, and even death. Local irritation is common, and inadvertent contact with the eye may cause severe conjunctivitis. Use during pregnancy is contraindicated in view of possible cytotoxic effects on the fetus. 

Adverse effects These are primarily related to CNS and Gl disturbances. They include impaired concentration, dizziness, ataxia, diplopia, somnolence, nervousness, and confusion, as well as nausea and weight loss. Renal stones have been reported in 1.5 percent of patients. Topiramate is teratogenic in animals, and should be avoided during pregnancy. Inducers of drug metabolism such as phenytoin and carbamazepine decrease topiramate serum concentrations by approximately 50 percent. Topiramate decreases ethinyl estradiol concentrations of oral contraceptive preparations, and individuals should supplement the amount of ethinyl estradiol. 

A 36-year-old woman with bipolar disorder who had taken lithium for 17 years continued to take it and other medications during pregnancy (493). At 35 weeks she developed signs of lithium toxicity, with nausea, diarrhea, and a concentration of 1.25 mmol/1. She delivered a lethargic infant with poor muscle tone, who showed signs of respiratory distress and hypopnea.
