To properly instruct the patient in administration of drug via a metered-dose inhaler, the nurse must be aware of general instructions for use for all metered-dose inhalers and three common methods of rise holding the lips around the mouthpiece, holding the inhaler away from the mouth, and rising a spacer or extender.

Corticosteroid Inhaled Powder—Hold the inhaler upright and twist off the cover. Twist the grip to the right as far as it will go, listen for the click, and then twist it back. Exhale and place the mouthpiece between lips slightly tilt head back and inhale deeply and forcefully. Remove inhaler from the mouth and hold breath for about 10 seconds. Rinse the mouth with water after each use to help reduce dry mouth and hoarseness. 

D NOTE Inhaled dry powder capsules require a different inhalation technique. To use a dry powder inhaler, it is important to close the mouth tightly around the mouthpiece of the inhaler and to inhale rapidly. 

Organic Lead. Following a single exposure to vapors of tetraalkyl lead compounds (approximately 1 mg/m3 breathed through a mouthpiece, 10-40 breaths of approximately 1 L volume) in four male subjects, 37% and 51% of inhaled tetraethyl and tetramethyl lead, respectively, were initially found in the respiratory tract, but a considerable percentage of these volatile compounds was lost through exhalation (Heard et al. 1979). Approximately 60-80% of the deposited tetraalkyl lead was absorbed by the lungs. 

The metered dose inhalers consist of four basic functional elements, container, metering valve, actuator and mouthpiece. 

Hovione s FlowCaps inhaler is a third example and is currently in development (Hovione, Portugal). Similar to Eclipse, the inhaler uses two blades that make cuts in the cap and body. The cuts are of unequal size, and this difference helps to fluidize the powder out of the capsule when the patient inhales. In the FlowCaps device, up to 14 capsules of size 4 can be loaded. One capsule at a time drops into the cutting area, an accurately sized tube guided by an inclined ramp, when the patient operates the device. Twisting the body relative to the mouthpiece cuts a slot into the capsule for release of its content into the inspiratory airflow. 

Breathe out and then put the mouthpiece into your mouth and take a deep inhalation, simultaneously pressing the inhaler. 

The delivery efficiency, or the nebulizer output, can be expressed in many different ways, as widely documented in the literature. Sometimes it is represented as the volume output, or solution mass output. However, it is more practical to use the drug mass emitted from the nebulizer at or near the mouthpiece to estimate the nebulizer output, because the amount of aerosolized drug mass at this point in the system is the best measure of how much drug the patient has available to inhale. 

Nebulizers fitted with a T-mouthpiece have an unrestricted flow of ambient air passing through the nebulizer output, supplying inhaled air flow, which effectively increases drug output. In vented nebulizers, the inhaled air must flow through the droplet production region. Therefore, the breathing pattern of the patient has an effect on the aerosol characteristics produced by such devices. The delivery efficiency of...

Different types of spacer device are available for use with metered dose inhalers but all work in basically the same manner. The spacer usually consists of two parts that slot together with a mouthpiece at one end and an opening for an inhaler at the other. There is a one-way valve which ensures that when a dose of the inhaler is expressed into the spacer the drug is stored in what is basically a holding device. This makes it easier to use a metered dose inhaler successfully as the two actions (pressing down on the metered dose inhaler and inhaling the medication) can be separated into two separate events. 

Remove the cap from the inhaler and insert the mouthpiece of the inhaler into the opening at the end of the spacer. 

Never wash the Turbohaler - if water or other fluid gets into the inhaler it may prevent it from working properly. The outside of the mouthpiece should be cleaned using a dry tissue. 

With the Accuhaler mouthpiece facing you, slide the lever away until it clicks. This will have loaded a dose ready for inhalation and...
