Mepivacaine hydrochloride

Mepivacaine hydrochloride [1722-62-9] similar in profile to Hdocaine, is used for infiltration, peripheral nerve blocks, and extradural anesthesia. It appears to be less toxic than Hdocaine in adults but more toxic in newborns. The duration of action is longer than that of Hdocaine because of its lower vasodilator activity. Mepivacaine has Htde topical activity. 

Mepivacaine hydrochloride (Carbocaine) is longer acting than lidocaine and has a more rapid onset of action (3-5 minutes). Topical application is not effective. It has been widely used in obstetrics, but its use has declined recently because of the early transient neurobehavioral effects it produces. Adverse reactions associated with mepivacaine are generally similar to those produced by other local anesthetics. It can be used with epinephrine or levonordefrin (dental use only). 

Dose. Mepivacaine hydrochloride is given by injection in doses of up to 400 mg maximum of 1 g in 24 hours.

A 71-year-old Japanese man developed an itchy erythematous papular eruption after using an over-the-counter medicament for skin wounds (Makiron) for 1 month (34). Patch tests with the constituents showed positive reactions to dl-chlorphenamine maleate and cinchocaine hydrochloride (both 1% in petrolatum). Patch tests with lidocaine hydrochloride and mepivacaine hydrochloride showed no cross-sensitization.

Sample preparation Dilute with water to a concentration of 833 pg/mL, add a 300 pL aliquot to 250 pL 2 mg/mL mepivacaine hydrochloride in water, vortex, ipject a 10 pL aliquot.

It may be prepared by adopting the same eourse of reactions as stated in for the synthesis of mepivacaine, hydrochloride except employing butyl bromide instead of dimethyl sulphate for the N-alkylation. 

Mepivacaine hydrochloride [N-(2, 6-dimethylphenyl)-1-methyl 2-piperidinecarboxamide monohydrochloride] is an amino amide-type local anesthetic agent widely used to provide regional analgesia and anesthesia by local infiltration, peripheral nerve block, and epidural and caudal blocks. The pharmacological and toxicological profile of mepivacaine is quite similar to that of lidocaine, except that mepivacaine has a slightly longer duration of action and lacks the vasodilator activity of lidocaine. For this reason, it serves as an alternate choice for lidocaine when addition of epinephrine is not recommended in patients with hypertensive vascular disease.
