Today, the hospital pharmacist is still mandated to control the supply chain, however there are challenges firom supply chain professionals supported by those who consider medicines to be no different firom any other commodity. Procurement now has to take place in an environment that has more and more budget restraints and external paralysing constraints induced by unwanted dependencies of third party suppliers and sometimes even drug shortages. If the skills are lost, important financial resources would be needed to reactivate lost know-how related to neglected technical equipment or outsourced activities. However, such time is not available in situations such as medicines shortages. Consequently, re-adaption may cost more than ever could have been saved. 

The most severe among a list of multifactorial reasons [21] which have induced a medicines shortage, were Quality or availability problems related to active ingredients or to production processes or equipment (e.g. heparin contamination [22] and propofol case [23]) Demand spikes (e.g. oseltamivir following flu pandemic scenarios [24])...

Drug-related problem is not a major subject in most university programmes in medicine or pharmacy. When there is no specific course, there is often no book covering the topic. In our view, as teachers at various university courses, there has been a shortage of literature that reflects the most important aspects of drug-related problems in the elderly. Medical practitioners, nurses and pharmacists, need to have this knowledge to be able to serve their patients in the best way.

Judged in terms of the relationship of benefit to cost, vaccines are among the most socially valuable public health investments (U.S. Centers for Disease Control and Prevention [CDC] 1999 Stratton, Durch, and Lawrence 2000)d In spite of some recent successes, such as increases in immunization rates in the United States (CDC 2002a, b, 2003), substantial structural and financial problems remain. In particular, the United States has recently experienced unprecedented shortages in 8 of the 11 routine childhood vaccines (Georges et al. 2003). Flu vaccine shortages were experienced in 2000-2002 and 2004 (Cohen 2002 Enserink 2004 Institute of Medicine 2004). 

For many technological applications such as in agriculture, medicine, and geology, the entity to be measured resides in a complex matrix of other entities. Composition and texture of that matrix may affect the measurement of the measurands. A call for RMs with all kinds of matrices and applicable measurands in many concentrations is not reasonable [1], There is an inevitable shortage of appropriate RMs with a matrix that adequately resembles that of the unknown sample, and there is little hope that this shortage will fade in time [14]. On the contrary, the ever-increasing complexity of our samples and the reduced uncertainties that will be needed will exacerbate the problem. The development and preferential choice of measurement methods that deliver matrix-independent results are more likely to lead to methodologies of wider applicability. Such methods should be seen as a major aim for chemical measurement research.
