Developing a Safety and Health Program, Second Edition 

A form should be developed to be used company-wide to document aU safety training. At a minimum, this form should consist of employees name. Social Security number, kind of training performed, date of training, and signature of trainer. This documentation can be housed in the employee s personnel file or in a computerized system.
