IgE-mediated allergic reactions

The initiation and maintenance of IgE antibody responses are regulated by, and dependent upon, the generation of T lymphocyte responses of the correct (Th2-type) phenotype. IgE-mediated allergic reactions (p. 72) involve mast cell release of histamine (p. 114) and production of other mediators (such as leukotrienes, p. 196). Resultant responses include relaxation of vascular smooth muscle, as evidenced locally by vasodilation (e.g., conjunctival congestion) or systemically by hypotension (as in anaphylactic shock) enhanced capillary permeability with transudation of fluid into tissues— swelling of conjunctiva and mucous membranes of the upper airways ( hay fever ), cutaneous wheal formation contraction of bronchial smooth muscle-bronchial asthma stimulation of intestinal smooth musde—diarrhea. 

Although bivalves are likely the most frequently ingested class of molluscan shellfish, the existence of allergic reactions to bivalves is rather poorly documented in the medical literature. IgE-mediated allergic reactions to oyster, clam, scallop, mussel, and cockle have been reported as described below. 

Bacitracin is indicated in prophylaxis and treatment of local infections, treatment of secondary pyodermas, as an adjunct in burn treatment, and as prophylaxis in operative wounds. However, it is not indicated in the treatment of chronic ulcers because of the increased risk of allergic reactions. There are several reports of delayed hypersensitivity, acute IgE-mediated allergic reactions, and anaphylactic reactions to bacitracin.51-53...

Atopy denotes a hereditary predisposition for IgE-mediated allergic reactions. Clinical pictures include allergic rhinoconjunctivitis ( hay fever ), bronchial asthma, atopic dermatitis (neurodermatitis, atopic eczema) and urticaria. Evidently, differentiation ofT-helper (TH) lymphocytes toward the TH2 phenotype is the common denominator. Therapeutic interventions are aimed at different levels to influence pathophysiological events (A). 

A non-IgE-mediated allergic reaction to bupivacaine has been reported (SEDA-21, 136). 

Hamburger pentapeptide, human IgE pen-tapeptide (HEPP), H-Asp-Ser-Asp-Pro-Arg-OH, a 5-peptide related to the human IgE sequence 330-334 inhibiting cutaneous and systemic IgE-mediated allergic reaction in humans [R.
