Heroin dealing

Because the reasons people use (and continue to use) drugs are complex and varied, treatment must address a challenging array of issues. Generally the first step after assessing a client s condition and situation is to deal with the physical aspects of the addiction. The papers dealing with the GC/MS detection of 10 different opiates in hair are listed in Table 7. The first report was published in 1984 about codeine detection in animal hair after administration of the drug. One paper is included with direct insertion of the probe and Cl. As heroin samples always contain codeine as an impurity, this substance also can be detected in cases of heroin abuse. Morphine is a metabolite of codeine and can be detected when codeine is abused. The quantitation of both drugs allows differentiation between codeine and heroin abuse. The detection of heroin or 6-acetylmorphine opens the possibility to prove directly the abuse of heroin.2 2" Here also, the more lipophilic 6-acetylmorphine exceeds the morphine in most samples. 

There is a paucity of literature on the comparison of concentrations of drug in head hair vs. arm hair. Cone et al. reported a study dealing with 20 paired head-and arm-hair samples collected from known heroin or cocaine abusers in which cocaine, heroin, and metabolites were analyzed simultaneously with GC/MS. Cocaine and 6-acetylmorphine were the major analytes present in both head (0.4-76 ng/mg and 0.0-0.8 ng/mg) and arm (0.0-109 ng/mg and 0.0-3.1 ng/mg) hair samples. Cocaine and benzoylecgonine concentrations tended to be higher in arm hair than in head hair samples, although the differences were not significant. 

The determination of the heroin metabolites morphine, morphine-6-glucuronide (M6G), morphine-3-glucuronide (M3G), and 6-monoacetyhnorphine (6-MAM) in body fluids is an important application of LC-MS in the toxicology laboratory. In some cases, codeine and codeine-6-glucuronide are determined as well. Several quantitative methods have been reported. However, only a few deal with forensic toxicology, performing the analysis in urine, serum, vitreous humour [79], and autopsy whole blood [79-80]. 

Political and social forces currently contribute to the demand for drug treatment, just as these same forces in the 1960s pushed the growth of treatment of heroin abuse. These trends reaffirm the importance of social and political factors in how U.S. society deals with alcohol and other drug use. 

An additional problem In dealing with opiate abuse Is that If opium prices should rise prohibitively, synthetic alternatives like methadone would be made available to sophisticated black market organizations. The most useful modality for dealing with the societal aspects of opiate abuse (as well as some of the medical and personal aspects) has been methadone maintenance. In consideration of the pharmacology of opiate dependence 4,17,45,58,87,88,131,140,172,185 Dole and Nyswander 51 proposed that maintenance of opiate addicts on relatively large doses of oral methadone would inhibit drug seeking behavior, "block" the subjective effects of parenteral heroin injection because of the tolerance induced, break the needle habit because of lack of reinforcement by this route, and make the addict more available for psychiatric treatment and rehabilitation, breaking the cycle of crime and punishment.
