Prophylactic ACh medications (but not amantadine) are reasonable when using high-potency FGAs (e.g., haloperidol, fluphenazine), in young men, and in patients with a history of dystonia. 

Hypersensitivity to the drug or any other component of the product (cross-sensitivity between phenothiazines may occur) comatose or greatly depressed states caused by CNS depressants or from any other cause (phenothiazines, clozapine, loxapine, molindone, pimozide, haloperidol) coadministration with other drugs that prolong the QT interval and in patients with congenital long QT syndrome or history of cardiac arrhythmias (mesoridazine, thioridazine, pimozide, ziprasidone see Drug..Interactions). 

HALOPERIDOL Individualize dosage. Children, debilitated, or geriatric patients and those with a history of adverse reactions to neuroleptic drugs may require less haloperidol. 

The majority of cases reported in the literature have occurred with CPZ and rarely with other agents such as promazine, thioridazine, mepazine, prochlorperazine, fluphenazine, and triflupromazine. There is no convincing evidence that haloperidol or other nonphenothiazine agents produce this type of jaundice. It is occasionally useful to obtain baseline liver function tests on patients with increased susceptibility (e.g., prior history of hepatitis) in the unlikely event that jaundice may develop. Routine serial liver function tests have never proven useful or necessary. The offending drug should be discontinued if a patient develops jaundice, but the value of this practice is also unproven, because many patients have been maintained on CPZ throughout an episode of jaundice without adverse consequences ( 509).

Patients taking olanzapine reported a low incidence of dystonias, which may be about 0.3% (SEDA-22, 56). In the light of two new cases of acute dystonia associated with olanzapine in patients with previous history of dystonia or parkinsonism related to antipsychotic treatment, comparative figures have been reported. Acute dystonia occurred in 1.4% of patients who took olanzapine, compared with 5.0-6.3% of those taking haloperidol (84).
