Gonadal hormone treatments

Hormonal therapies have shown activity in the treatment of cancers whose growth is affected by gonadal hormonal control. Hormonal treatments either block or decrease the production of endogenous hormones.

Saw palmetto is most often used in the treatment of benign prostatic hyperplasia. Enzymatic conversion of testosterone to dihydrotestosterone (DHT) by 5a-reductase is inhibited by saw palmetto in vitro. This effect is similar to that of finasteride, which is also used to treat the disorder (Chapter 40 The Gonadal Hormones Inhibitors). In vitro, saw palmetto also inhibits the binding of DHT to androgen receptors. Additional effects that have been observed in vitro include inhibition of prostatic growth factors, blockade of ay-adrenoceptors, and inhibition of inflammatory mediators... 

Hyperprolactinemia can lead to hypogonadism secondary to prolactin s inhibitory effects on the hypothalamic-pituitary-gonadal hormonal axis. A 35-year-old female with psychosis and history of 7years of treatment with different antipsychotics presented with hypogonadism and radiological evidence of a pituitary microadenoma while on amisulpride and aripiprazole [53 ]. The authors did not relate the microadenoma to the hyperprolactinema and commented on the difficulty of treating these disorders.

The male and female gonads, as well as the placenta of pregnant females and, to a lesser extent, the adrenal cortex, produce a range of steroid hormones which regulate the development and maintenance of reproductive and related functions. As such, these steroid sex hormones have found medical application in the treatment of various reproductive dysfunctions. 

Ketoconazole (an antifungal agent, see p. 340) strongly inhibits all gonadal and adrenal steroid hormone synthesis. It is used in the treatment of patients with Cushing s syndrome. 

Inhibin, an ovarian protein hormone, has been shown to selectively decrease FSH but not LH secretion by the anterior pituitary cell.
