Fluocinonide cream

Fluocinonide (50 mg) is dissolved in crotamiton (7 g) with warming, and thereto is added liquid paraffin (10 g), propylene glycol (10 g), polyoxyethylene lauryl ether (1 g), a 4% aqueous solution of carboxyvinyl polymer (20 g), purified water (47 g), and a 1% aqueous solution of disodium edetate (1.2 g).

A 35-year-old woman had a 3-week history of pruritic erythema, edema, and linear vesiculation of the upper arms where she had applied fluocinonide cream (1). Patch tests with the cream were positive, but the active ingredient fluocinonide (0.05% in petrolatum) was negative, while 1,2,6-hexanetriol 5% showed strong positive reactions on days 3 and 7.

The cream contains fluocinonide 0.5 mg/g in a specially formulated cream base consisting of citric acid, 1,2,6-hexanetriol, polyethylene glycol 8000, propylene glycol, and stearyl alcohol. This white cream vehicle is greaseless, nonstaining, anhydrous, and completely water miscible. The base provides emollient and hydrophilic properties. In this formulation, the active ingredient is totally in solution. 

The cream contains fluocinonide 0.5 mg/g in a water-washable aqueous emollient base of cetyl alcohol, citric acid, mineral oil, polysorbate 60, propylene glycol, sorbi-tan monostearate, stearyl alcohol, and water (purified). 

Fluocinonide, a topical adrenocorticoid with antiinflammatory properties (0.05% cream, gel, ointment, solution), is used in inflammation of corticosteroid-responsive dermatoses (see also Table 14). 

Sample preparation Dissolve 5 g cream containing 0.00925% fluocinonide, 0.00365% pro-cinonide, and 0.0021% ciprocinonide in 2.5 mL THF, add norethindrone, lute to 25 mL with MeOH, centrifuge, inject a 25 pL aliquot onto column A with mobile phase A and allow components to elute from column A to column B for 7 min. After 7 min remove column A from circuit, monitor effluent from column B. Back-flush column A with mobile phase B for 5 min, equilibrate column A with mobile phase A for 5 min before next injection.

As an example, fluocinonide is a prodrug that contains an acetal moiety, and is sold in a cream used for the topical treatment of eczema and other skin conditions.
