Failure to sense

As with all manmade devices there is the possibility that the device can be faulty, either due to a fault in the manufacturing process or in the programming of the device. Pacemaker malfunctions can often be seen on the 12-lead ECG. The most commonly encountered problems associated with pacemaker systems include failure to sense, failure to capture, failure to pace and over sensing. 

One of the procedures used to determine which sensors are needed to sense process conditions and protect the process is called a Failure Mode Effect Analysis—FMEA. Every device in the process is checked for its various modes of failure. A search is then made to assure that there is a redundancy that keeps an identified source or condition from developing for each potential failure mode. The degree of required redundancy depends on the severity of the source as previously described. Table 14-2 lists failure modes for various devices commonly used in production facilities. 

Other sensors are mostly grouped towards the triethylamine. In the case of porphyrins 6-8, 13 the coordinated metal is no longer able to drive the selectivity pattern and the presence of the peripheral alkyl chains completely shadows the coordination interactions. This result can explain the failure to observe the coordination interaction in the sensing mechanism of the metal complexes of the closely related alkyl chains functionalized phthalocyanines reported in the past by Gopel and coworkers [22]. 

A considerable reduction in pumping speed and failure to reach the ultimate pressure which is normally attainable in spite of thermal regeneration having been carried out indicates that the zeolite being used has become contaminated by outside substances. It does not make good sense to attempt to rejuvenate the contaminated zeolite with special thermal processes. The zeolite should simply be replaced. 

Redundancy. The existence of more than one means for accomplishing a given task, where all means must fail before there is an overall failure to the system. Parallel redundancy applies to systems where both means are working at the same time to accomplish the task, and either of the systems is capable of handling the job itself in case of failure of the other system. Standby redundancy applies to a system where there is an alternate means of accomplishing the task that is switched in by a malfunction-sensing device when the primary system fails Ref Anon, OrdTechTerm (1962), 251—52...

There ore operations of the law of nature which are not patentable. On the other hand, there have been discoveries made in scientific fields which in a sense are phenomena of old and well-known products. These are not described as laws of nature. Problems arise chiefly in relationship to what already is known. Failure to distinguish between product coverage in this area resulted in a categorical application of the rule to both situations. This has occasioned much of the misunderstanding. The courts never intended the meaning of the rule which would declare otherwise patentable compositions of matter to be unpatentable merely because their creation might be the handiwork of nature rather than of man. 

A 37-year-old man with paranoid schizophrenia had ejaculatory difficulty during sexual intercourse with his wife, compatible with retrograde ejaculation, 1-2 weeks after starting to take risperidone (201). He reported complete failure to emit semen but a normal desire, erection, and sense of orgasm. Semen was seen in postcoital urine. The dosage of risperidone was reduced to 3 mg/day and anterograde ejaculation was partially restored. 

Major Pharma doesn t like to distribute its data on failure to stock market analysts, lest they pull down their ratings, but they can give them to a common—in the sense of shared—consultant. The combined data from the top companies (see Table 15.1) show that failure is industrywide no one player in the industry is significantly better than the other. But different indications have different failure rates for different reasons, but failure rates are not different between the companies for the same types of indications. 

Once the plans have been formulated, they should be tested and documented to see if they work. Failure to test the recovery plan will give a false sense of security and compound any disaster. 

In phenomenological non-equilibrium thermodynamics a complicated system is described by taking each flux as dependent on each of the forces within the system. On the other hand, common sense tells us that some processes will in practice not depend noticeably on some of the forces. In fact, it could almost be considered a definition of an independent process that it is independent of driving forces other than its own driving force. Thus, we can simplify the description by splitting it up into independent processes. It should always be remembered, however, that such independence of processes remains a postulate of the description failure to fit the properties of the system with the equations may be the result of an unnoticed coupling.
