Retrograde ejaculation is a complication of up to 75% of transurethral prostatectomy procedures. Other complications seen in 2% to 15% of patients include bleeding, urinary incontinence, and erectile dysfunction. 

All conventional antipsychotic medications and risperidone may cause hyperprolactinemia. Side effects mediated, at least in part, by hyperprolactinemia include gynecomastia, galactorrhea, amenorrhea, and decreased libido. Thioridazine may cause painful retrograde ejaculation. 

Adverse effects include dizziness and retrograde ejaculation. 

Seminal emission and ejaculation are under control of the sympathetic nervous system. Emission results from a-adrenergic-mediated contraction of the epididymis, vas deferens, seminal vesicles, and prostate, which causes seminal fluid to enter the prostatic urethra. Concomitant closure of the bladder neck prevents retrograde flow of semen into the bladder, and antegrade ejaculation results from contraction of the muscles of the pelvic floor including the bulbocavemosus and ischiocavernosus muscles. 

Orgasm is a psychosensory phenomenon in which the rhythmic contraction of the pelvic muscles is perceived as pleasurable. Orgasm can occur without either erection or ejaculation or in the presence of retrograde ejaculation (Gingell and Lockyer, 1999). 

Retrograde ejaculation has been associated with clozapine (201).

A 37-year-old man with paranoid schizophrenia had ejaculatory difficulty during sexual intercourse with his wife, compatible with retrograde ejaculation, 1-2 weeks after starting to take risperidone (201). He reported complete failure to emit semen but a normal desire, erection, and sense of orgasm. Semen was seen in postcoital urine. The dosage of risperidone was reduced to 3 mg/day and anterograde ejaculation was partially restored. 

Risperidone may have caused retrograde ejaculation by altering sympathetic tone and allowing semen to pass retrogradely into the bladder during ejaculation.

The detrusor, whose smooth muscle fibres comprise the body of the bladder, is innervated mainly by parasympathetic nerves which are excitatory and cause the muscle to contract. The internal sphincter, a concentration of smooth muscle at the bladder neck, is well developed only in the male and its principal hmction is to prevent retrograde flow of semen during ejaculation. It is rich in aj-adrenoceptors, activation of which causes contraction. There is an abvmdant supply of oestrogen receptors in the distal two-thirds of the female urethral epithelium which degenerates after the menopause causing loss of urinary control.
