Endocarditis surgery

Baddour LM, Wilson WR, Bayer AS, et al. Infective endocarditis Diagnosis, antimicrobial therapy, and management of complications a statement for healthcare professionals from the Committee on Rheumatic Fever, Endocarditis, and Kawasaki Disease, Council on Cardiovascular Disease in the Young, and the Councils on Clinical Cardiology, Stroke, and Cardiovascular Surgery and Anesthesia, American Heart Association endorsed by the Infectious Diseases Society of America. Circulation 2005 111(23) 394M34. [Pg.1032]

No pathologic evidence of infective endocarditis at surgery or autopsy, with antibiotic therapy for less than or equal to 4 days, or... [Pg.1094]

Fungal endocarditis is quite uncommon but has significant mortality, typically affecting patients who have had cardiovascular surgery, received a prolonged course of broad-spectrum antibiotics, have long-term catheter placement, are immunocompromised, or are IVDUs.8,35 Survival rates have remained... [Pg.1095]

Surgery is an important adjunct to management of endocarditis in certain patients. In most cases, valvectomy and valve replacement are performed to remove infected tissues and restore hemodynamic function. The most important indications for surgical intervention in the past have been heart failure in left-sided IE and persistent infections in right-sided IE. [Pg.414]

S. aureus has become more prevalent as a cause of endocarditis because of increased IV drug abuse, frequent use of peripheral and central venous catheters, and valve-replacement surgery. Coagulase-negative staphylococci (CNST, usually S. epidermidis) are prominent causes of PVE. [Pg.416]

Even with this somewhat stricter definition, there is room for discretion. A sportsman who takes an occasional puff of a bronchodilator for exercise-induced asthma but is otherwise asymptomatic may be considered eligible by some. Individuals who have undergone surgery for a congenital condition and are in excellent health may or may not be suitable. Thus, an asymptomatic patient with a hip prosthesis who is taking no medication may be acceptable whereas an equally healthy individual with a prosthetic heart valve should be excluded from a study involving a cannula because of the risk, however remote, of endocarditis. Clearly, whatever definition of a healthy volunteer is used, sensible clinical judgement is still required. [Pg.154]

Severe renal impairment (Ccr less than 30 mL/min) fondaparinux as prophylactic therapy in patients with body weight less than 50 kg undergoing hip fracture, hip replacement, or knee replacement surgery active major bleeding bacterial endocarditis thrombocytopenia associated with a positive in vitro test for antiplatelet antibody in the presence of fondaparinux known hypersensitivity to fondaparinux. [Pg.166]

Oral anticoagulants are ordinarily contraindicated in the presence of active or past gastrointestinal ulceration thrombocytopenia hepatic or renal disease malignant hypertension recent brain, eye, or spinal cord surgery bacterial endocarditis chronic alcoholism and pregnancy. These agents also should not be prescribed for individuals with physically hazardous occupations. [Pg.261]

Treatment of Bone, respiratory tract, skin and soft-tissue infections, endocarditis, peritonitis, and septicemia prevention of bacterial endocarditis in those at risk (if peniciiiin is contraindicated) when undergoing biliary, dental, GI, GU, or respiratory surgery or invasive procedures IV 500mgq6hor lgql2h. [Pg.1297]

Various antimicrobials agents is used in dentistry for prevention of local oral wound infection and prevention of distant infection i.e., bacterial endocarditis. Generally, prophylaxis by the use of antibiotics is not required for routine type of dental surgery... [Pg.303]

Examples of chemoprophylaxis in the non-surgical arena include the prevention of endocarditis with amoxicillin in patients with valvular heart disease undergoing dental surgery, and the prevention of secondary cases of meningococcal meningitis with rifampicin among household contacts of an index case. [Pg.238]

Cautions Recent major surgery (Coronary artery bypass graft, OB delivery, organ biopsy), cerebrovascular disease, recent GI or GU bleeding, hypertension, mitral stenosis with atrial fibrillation, acute pericarditis, bacterial endocarditis, hepatic/renal impairment, diabetic retinopathy, ophthalmic hemorrhaging, septic thrombophlebitis, occluded AV cannula at an infected site, advanced age, those receiving oral anticoagulants... [Pg.311]

Although prosthetic heart valves have been extensively and successfully anployed clinically, problems still exist during the postoperative period concerning thromboembolism. Infection and valvular Incompetence. Infectious prosthetic valve endocarditis (PVE) remains as a serious and potentially devastating consequence of valve replacement surgery with mechanically fabricated... [Pg.86]

Metronidazole has been shown to be of great value in the management of anaerobic bacterial infections [20,27,75,76]. The role of this drug in the prophylaxis and treatment of various anaerobic bacterial infections, which may develop following appendectomy, elective colonic surgery, colo-rectal surgery and hysterectomy [20,77,78]. Mebendazole is equally useful in cases of endocarditis, osteomyelitis, lung abscess, empyema, peritonitis, septicemia and pelvic infections [79]. [Pg.432]

In the past, rheumatic heart disease was a prevalent risk factor for IE, but the incidence of this disease continues to decline. The risk of IE in persons with mitral valve prolapse and regurgitation is small however, because the condition is prevalent, it is an important contributor to the overall number of IE cases. Prosthetic valve endocarditis occurs in 1 % to 4% of patients undergoing valve replacement surgery. ... [Pg.1998]
