Enter the 9-digit number assigned by Dun and Bradstreet (D B) for your facility or each establishment within your facility. These numbers code the facility for financial purposes. This number may be available from your facility s treasurer or financial officer. You can also obtain the numbers from your local Dun and Bradstreet office (check the telephone book White Paoest. If none of your establishments has been assigned a D B number, enter not applicable, NA, in box a. If only some of your establishments have been assigned Dun and Bradstreet numbers, enter those numbers in Section 3.7. 

Enter the Dun and Bradstreet Number for your ultimate US parent company. If applicable. The number may be obtained from the treasurer or financial officer of the company. If your parent company does not have a Dun and Bradstreet number, enter not applicable, NA.

Each report must contain the company name and Dun and Bradstreet Number the name, mailing address, phone number, and e-mail address of a technical contact and for each plant site, the name and full street address of each site. Is the establishment/worksite, or the company that owns the establishment or uses the worksite, owned by another legal entity (parent company) If so, what is the name and location Try to find out whether the inspected establishment/worksite is a division or a subsidiary of the parent company. (NOTE A division is a wholly-owned part of the same company that may be differently named, e.g., Chevrolet is a division of GM. A subsidiary is a company controlled or owned by another company which owns all or a majority of its shares. Try to determine if the parent company has divisions or subsidiaries other than the one that owns or uses the establishment or worksite being inspected. If so, try to get the names and the type of business they are involved in. Sometimes this type of information can be found on a website or in Dun and Bradstreet. Another good source of information is the office of the Secretary of State within the state government. 

Financial vitality Supplier s financial strength is monitored with published data from Dun and Bradstreet (www.dnb.com) and other sources. 

An online interactive, global search tool is designed to help product manufacturers, designen, and specifiers find and qualify exacdy the right supplier. Internet users worldwide can search the Supplier Search centralized database for free. Technically detailed templates and a powerful search engine allow users to identify and communicate with firms that meet their specific parameters in terms of process type, capabilities, and location. Several sections also include links to Dun Sc Bradstreet reports on the companies. 

Robert Morris Associates also compiles extensive comparative company data for various industries. In addition to ratios similar to the Dun Bradstreet ratios shown in Table 9-24, Robert Morris Associates gives very useful breakdowns of assets and habilities for various industries. Table 9-25 shows a breakdown of assets and liabihties for United States manufacturers of industrial inorganic chemicals. 

There is a formula for business failures based on Dun Bradstreet, Inc. annually published data. The vast majority of the firms involved are small. Why do failures occur D B has offered the following tabular explanation (apparent cause/percent) inadequate sales/49.9, competitive weakness/25.3, heavy operating expenses/13.0, receivables difficul-ties/8.3, inventory difficulties/7.7, excessive fixed assets/3.2, poor location/2.7, neglect/0.8, disaster/0.8, fraud/0.5, and others/1.1. Numbers do not add up to 100% because some failures are attributed to a combination of apparent causes. One can include that product design directly influences competitive weakness and heavy operating expenses. 

Data on private companies are difficult to assemble, and financial checking on the CROs is not straightforward. When published at all, these companies accounts are often in abbreviated format, and can be out of date. Organizations such as Dun Bradstreet may be able to provide useful information. However, it is likely that the best source of reassurance of financial stability of a CRO is via bankers references, obtained through the sponsor s own finance department. 

Consult Dun Bradstreet Million Dollar Directory and Who s Who in Finance and Industry and find the name of the CEO or president of Air Products and Chemicals, Du Pont, Merck, Union Carbide, and Exxon. Give a brief profile of each coiporate leader. 

Within the first two groups we then made a distinction between mother companies and subsidiaries. In order to distinguish firms t) ology and sector, the Who Owns Whom 1997 directory (WOW, Dun Bradstreet) was used.
