Percutaneous absorption is variable and may result in significant drowsiness in some patients. In view of the anticholinergic effect of doxepin, topical use is contraindicated in patients with untreated narrow-angle glaucoma or a tendency to urinary retention. [Pg.1305]

Topical doxepin hydrochloride 5% cream (Zonalon) may provide significant antipruritic activity when utilized in the treatment of pruritus associated with atopic dermatitis or lichen simplex chronicus. The precise mechanism of action is unknown but may relate to the potent Hi and H2-receptor antagonist properties of dibenzoxepin tricyclic compounds. Percutaneous absorption is variable and may result in significant drowsiness in some patients. In view of the anticholinergic effect of doxepin, topical use is contraindicated in patients with untreated narrow-angle glaucoma or a tendency to urinary retention. [Pg.1465]

The tricychc tertiary amine doxepin Zonalon, Prudox), a potent Hj- and Hj-receptor antagonist, is indicated for the short-term relief of pruritus associated with topical eczematous dermatitis. Systemic absorption occurs, and the drug may potentiate or alter the metabolism of a va-... [Pg.494]

Topical doxepin in the form of cream is used in the treatment of pruritus associated with atopic dermatitis or lichen chronicus. Adverse effects include marked burning and stinging at the treatment site. [Pg.453]

Plasma levels of doxepin similar to those achieved during oral therapy may be obtained with topical application the usual drug interactions associated with tricyclic antidepressants may occur. Therefore, monoamine oxidase inhibitors must be discontinued at least 2 weeks prior to the initiation of doxepin cream. Topical application of the cream should be performed four times daily for up to 8 days of therapy. The safety and efficacy of chronic dosing has not been established. Adverse local effects include marked burning and stinging of the treatment site which may necessitate discontinuation of the cream in some patients. Allergic contact dermatitis appears to be frequent, and patients should be monitored for symptoms of hypersensitivity. [Pg.1305]

Doxepin is a tricyclic antidepressant that has also been used topically in the treatment of atopic dermatitis and other forms of eczematous dermatitis. It causes the adverse effects that one would expect, and systemic effects can result from absorption after topical administration. [Pg.33]

Calamine and astringents (aluminium acetate, tannic acid) may help. Local anaesthetics do not offer any long-term solution and since they are liable to sensitise the skin they are best avoided lignocaine is least troublesome in this respect. Topical doxepin... [Pg.302]

One tricyclic antidepressant, doxepin, inhibits both Hi and H2 receptors, and it has been used for treating adults with AD in doses of 10 to 75 mg at night and up to 75 mg twice daily. This may be beneficial in those atopic patients who have some depression as well. ° Topical antihistamines, such as 5% doxepin cream or diphenhydramine cream, also have demonstrated neutral results, but are generally not recommended because of high cutaneous sensitization from excipient ingredients in the formulation. ... [Pg.1789]

Peak concentrations of these drugs are achieved rapidly in the skin and persist after plasma levels have declined. This is consistent with inhibition of wheal and flare responses to the intra-dermal injection of histamine or allergen, which last for 36 hours or more after treatment, even when concentrations in plasma are very low. Such results emphasize the need for flexibihty in the interpretation of the recommended dosage schedules (Table 24-2) less frequent dosage may suffice. Doxepin, a tricyclic antidepressant (see Chapter 17), is one of the most potent antihistamines available it is -800 times more potent than diphenhydramine. This may account for the observation that topical doxepin can be effective in the treatment of chronic urticaria when other antihistamines have failed. [Pg.407]

Five percent topical doxepin has been introduced as an antipruritic. Percutaneous absorption frequently leads to clinical sedation (package insert, Zonulong, Gen-derm, Lincolnville). [Pg.46]

Doxepin hydrochloride capsules (most commonly 25 and 50 mg) are used but oral solutions exist as well as concentrates at 10 mg/mL. It consists of a mixture of E- and Z-isomers (85 15). Strong interactions with histamine Hj and H2 receptors are the basis for its use in the topical treatment of severe pruritus. [Pg.363]
