Direct Payments

Households have direct and indirect costs. Direct household costs imply payments for transport to and from the health services, acconunodation for the accompanying relative, special buildings for disabled patients (e.g., changed bathroom), costs for diet, and the re-education, for instance new training after a paralysis. Private households also have to bear direct payments for user fees and drugs, which are an income of the providers. There must be clearing between these components of the COI to avoid double-counting. 

In EU countries, the principle behind directives for collection, recovery, and reuse has been EPR, making producers responsible for the take-back of e-waste. Canada and Australia are among other countries developing systems based on these principles. Japan s Reuse, Recycling and Recovery system differs in some ways, while still promoting take-back by manufacturers. The most salient difference is the direct payment of recycling costs by Japanese consumers.

Competition for a limited pool of patients has led to recruitment and retention problems and to the need for direct payments to subjects for their participation.

In 1960,49% of health care revenues came from out-of-pocket payments from individuals. Out-of-pocket spending is defined as expenditures for coinsurance and deductibles required by insurers, as well as direct payments for services, which are covered by a third party. In 1990, individual consumers spent 144.4 billion ectly for out-of-pocket payments for personal health ser-vices.f This accounted for 38% of all personal health spending. In 1998, consumers spent 183.7billion in out-of-pocket payments, which accounts for 33% of the 558.7 billion in personal health spending. ... 

Nevertheless, within mainstream agricultural measures, part of the funding for direct payments will be allocated to national envelopes which may be distributed by member states according to local needs. Resources from these national envelopes could be used to support particular production systems, such as organic beef production (Lampkin and Midmore 2000). 

Simplified systems for implementing direct payments allow decoupled payments for a transitional period if the administrative structures to manage the complicated EU system are not yet in place. This decoupled approach resembles elements of the European Commission s mid-term review proposals (EC 2002b). As in the existing EU, the new members will also benefit from rural development measures which could be financed by the EU up to 80 per cent. Relative to the budget of the first pillar of the CAP, the budget for rural development in the new member states will be higher than in the old member states. 

Supporting advisory services for organic farmers and producers as the preferred management approach in specific regions may be as important as direct payments.

The latest reform of the CAP, Agenda 2000, is continuing the gradual shift from price support to direct payments. Especially price protection for livestock products will decrease in the future. However, since payments will continue to be paid per head (or, in the case of milk, will possibly even be directly coupled to production), the impact on the relative competitiveness of organic farming will be marginal.
