This review is essentially a continuation of the same chapter in the 1965 Annual Reports. Its primary purpose is to extend and up-date the material presented there. Both reviews emphasize physiology and biochemistry because the significant advances in diabetes research are being made on these fronts, while progress toward new antidiabetic drugs has been slow. This emphasis also stems from a conviction that new approaches to antidiabetic therapy are most likely to come from application of new knowledge about metabolism and its regulation.

Being able to monitor glucose without obtaining blood samples has opened up the possibility of doing diabetes research on small rodents which previously could be done only on larger animals. 

Recent reviews " have discussed many aspects of insulin synthesis, release and effects on metabolism. It is the purpose of this chapter to highlight more recent advances in the above areas as well as recent concepts regarding the etiology of diabetic complications. In addition we will briefly examine current methods of therapy. Since the major developments in diabetes research have been biological in nature, we have chosen to concentrate our review in this area. 

Uric acid is an alkaloid possessing a purine skeleton, and through oxidation, alloxan with a pyrimidine skeleton is obtained, as first reported in 1818. Subsequently it was established that alloxan possessed the ability to cause hyperglycemia experimentally in mice, and this alkaloid is used for this very important purpose in experimental models for diabetes research.
