Renal function impairment Renal impairment, including minimal change nephropathy, and acute and chronic interstitial nephritis, has occurred. 

Both lithium and demeclocycline have been reported to cause acute renal failure. Long-term lithium therapy may also cause chronic interstitial nephritis. 

Extensive literature has accumulated concerning other forms of renal dysfunction during long-term lithium therapy, including chronic interstitial nephritis and minimal-change glomerulopathy with nephrotic syndrome. Some instances of decreased glomerular filtration rate have been encountered but no instances of marked azotemia or renal failure. 

A 48-year-old man taking lithium and chlorprothixene had a creatinine clearance of 60 ml/minute and a renal biopsy showing chronic interstitial nephritis (380).

Two patients with long-standing ulcerative colitis developed what seemed to be a drug-induced chronic interstitial nephritis after taking sulfasalazine for several years, with no other detectable cause (73) the same problem has arisen with mesalazine. However, in a long-term stndy (mean treatment time 10 years) in 36 patients taking sulfasalazine for ulcerative cohtis, there was no nephrotoxicity (74).

Since toxin induced chronic renal failure is theorized to occur after years of low-level toxin exposure, it stands to reason that the incidence would be clustered in elderly patients. The study of Chester et al. [172] provides indirect support that elderly patients may be at greater risk. Of the 79 patients with chronic renal failure who met age criteria of 70 year or more, 29% were classified as having chronic interstitial nephritis, a clinical diagnosis quite compatible with toxin induced renal failure and an incidence substantially higher than the 10.4% in accumulated series in which patients 50 year and older were included [173]. Furthermore, in the 2006 USRDS survey of causes of ESRD, the average age for patients with a diagnosis of interstitial nephropathy was 56 year compared to 58 years for the entire population reported [119]. In the recent analysis of the PICARD database, advanced age was associated with increased mortahty in AKI patients both at time of consultation and after initiation of dialysis treatment [38].

The major pathologic presentation of solvent associated nephropathy is that of anti-glomerular basement membrane disease [39] but epimembraneous and subacute proliferative glomerulonephritis have also been demonstrated. In addition, Narvarte et al. [40] reported on a patient with ulcerative colitis in which chronic interstitial nephritis developed that later was attributed to long-term exposure to organic solvents.
