Although not stressed by the regulations, the objective of the incident investigation should be the development and implementation of recommendations to ensure the incident is not repeated. The IJ.S. Congress funded the board at 4 million for 1988. One key assumption, as stated in the Overview of the Chemical Safety Board s operations, is that there would be 330 catastrophic accidents annually. It was further assumed that within this total, between 10 and 15 of these accidents would be major catastrophic accidents with an average of two deaths per incident. The Chemical Safety Board has made recommendations that the scope of the standard be expanded with regard to reactive chemicals. OSHA s suggestion is to use the New Jersey TCPA as an example and also to follow the approach used by the Chemical Safety Board, which is performance based. [Pg.98]

The principal role of the U.S. Chemical Safety Board is investigating accidents to identify the conditions and circumstances that led to the events. It also investigates hazardous conditions that could lead to accidents. The agency uses root cause analysis and other methods to identify causes to prevent events. It makes recommendations to government agencies, companies, trade associations, labor unions, and other groups. It publishes reports and offers DVDs about cases it investigates. [Pg.44]

The Chemical Safety Board (CSB) investigation report identified seven significant facility and operational factors that had contributed to the incident.
