Although not stressed by the regulations, the objective of the incident investigation should be the development and implementation of recommendations to ensure the incident is not repeated. This objective should apply not only to the process involved, but also to all similar situations having the same potential. In major incidents, the Chemical Safety Board s investigation of reports serves as a vehicle to communicate to a much broader audience than the organizations that had the incident.

The IJ.S. Congress funded the board at 4 million for 1988. One key assumption, as stated in the Overview of the Chemical Safety Board s operations, is that there would be 330 catastrophic accidents annually. It was further assumed that within this total, between 10 and 15 of these accidents would be major catastrophic accidents with an average of two deaths per incident. [38] The industry must perform better than the predicted 330 catastrophic accidents for the public to fully earn our trust.

The Chemical Safety Board has made recommendations that the scope of the standard be expanded with regard to reactive chemicals. OSHA s suggestion is to use the New Jersey TCPA as an example and also to follow the approach used by the Chemical Safety Board, which is performance based. 

All organizations can learn from the experience of others. In spite of the litigious atmosphere in which the process industries work, information on accidents is available. Organizations such as the Center for Chemical Process Safety (CCPS) and the Chemical Safety Board (CSB) publish information, as do industry trade groups and specialist groups at professional meetings. 

Most credible initiating events and their fi equencies can be gathered either from the company s incidents library or from agencies such as CSB (Chemical Safety Board in US), CIMAH (Control of Industrial Major Accident Hazards in US), COMAH (Control of Major Accident Hazards in UK), European Seveso H Directive and HSE (Health and Safety Executive in UK). 

The principal role of the U.S. Chemical Safety Board is investigating accidents to identify the conditions and circumstances that led to the events. It also investigates hazardous conditions that could lead to accidents. The agency uses root cause analysis and other methods to identify causes to prevent events. It makes recommendations to government agencies, companies, trade associations, labor unions, and other groups. It publishes reports and offers DVDs about cases it investigates. 

The Chemical Safety Board (CSB) investigation report identified seven significant facility and operational factors that had contributed to the incident. One of those was siting support trailers for workers too close to the highly hazardous process unit. All fatalities occurred in and around the trailers.
