Larotaxel 59129>130 js a semi-synthetic derivative of 10-deacetyl baccatin III with a docetaxel-like side chain that has a low affinity for the P-glycoprotein drug efflux pump, an efflux mechanism that diminishes the effectiveness of the marketed drugs paclitaxel 60 and docetaxel. Importantly, this low affinity should enable larotaxel 59 to be effective in tumours resistant to paclitaxel 60...

Fatal cholestatic hver failure occurred in a 45-year-old woman with metastatic breast cancer who was given gemcitabine and carboplatin and pre-existing liver damage. After four courses of gemcitabine -I- carboplatin she developed severe decompensated cholestatic hepatitis (9). Liver biopsy showed marked cholestasis and hepatocellular injury consistent with drug-induced hepatotoxicity.

The pharmacokinetics of gemcitabine and doxorubicin did not differ when they were given on the same day, when compared with when they were given alone in patients with breast cancer. Similarly, gemcitabine pharmacokinetics were unchanged by the concurrent use of epirubicin and paclitaxel in patients with breast cancer, and gemcitabine did not alter the interaction between epirubicin and paclitaxel (see Anthracyclines -i-Taxanes , p.612). 

Cardiovascular Cardiac toxicity is rare. Acute myocardial infarction has been reported in patients with underlying cardiac disease [41" ]. Cardiac dysrhythmias are very rare [42" ]. In one case a 67-year-old woman with breast cancer and no cardiac history or risk factors developed a supraventricular tachycardia after an infusion of gemcitabine [43" ]. The dysrhythmia was eventually terminated with oral propranolol and no alternative precipitating cause of the event could be identified. There were no further episodes after withdrawal of the drug.
