SKF 525-A inhibits the in vitro metabolism of several barbiturates, meperidine, aminopyrine, and codeine, the formation of morphine glucuronide, and the hydrolysis of procaine and ethylglycinexylidide. However, SKF 525-A has no significant effect on the microsomal N-demethylation of monomethyl-4-aminoantipyrine, N-methylaniline or 3-methyl-4-monomethylaminoazobenzene, the O-dealkylation of phenacetin, the hydroxylation of acetanilide, or the sulphoxidation of chlorpro-mazine. [Pg.608]

Frequently, a preoperative sedative is given to a patient 1 to 2 hours before the administration of general anesthesia.2,36 Sedatives are usually administered orally or by intramuscular injection, and are given while the patient is still in his or her room. This approach serves to relax the patient and reduce anxiety when arriving at the operating room. Commonly used preoperative sedatives include barbiturates (secobarbital, pentobarbital), opioids (butorphanol, meperidine), and benzodiazepines (diazepam, lorazepam) (Table 11-2). The use of opiates in obstetrical analgesia is a highly specialized field requiring considerable experience and sound judgment to ensure safety. Morphine has been combined with scopolamine in twilight sleep, but this mixture is not used nearly as much as formerly. Various combinations of meperidine, barbiturate, scopolamine, paraldehyde, and the inhalation anesthetics have made morphine less popular in obstetrical work. The opiates are powerful respiratory depressants. The fetus is especially susceptible to morphine, which greatly increases the incidence of asphyxia in the newborn. Morphine and its derivatives are particularly contraindicated in premature labor because of the untoward effect of such medication on the premature infant. [Pg.457]

Naloxone (Narcan) and naltrexone hydrochloride (Trexan) reverse the respiratory depressant action of narcotics related to morphine, meperidine, and methadone. They differ from other narcotic analgesics in several respects. Naloxone does not cause respiratory depression, pupillary constriction, sedation, or analgesia. However, it does antagonize the actions of pentazocine. Naloxone neither antagonizes the respiratory depressant effects of barbiturates and other hypnotics nor aggravates their depressant effects on respiration.
