Unlike dendritic keratitis, indolent ulcers are typically very difficult to treat. Instillation of a prophylactic antibiotic, such as polymyxin B-bacitracin ointment two to four times a day, and a cycloplegic agent, such as 5% homatropine two to three times a day, is indicated. Therapeutic soft contact lens use with appropriate antibiotic therapy can also be considered as alternatives. These patients must be monitored carefully to ensure that no secondary infection develops. If the ulcer deepens, a new infiltrate forms, or if there is an increase in the anterior chamber reaction while the patient is being treated, cultures should be performed to rule out bacterial or fungal infection. Cyanoacrylate glue, conjunctival flap surgery, or tarsorrhaphy may be required if healing does not occur. 

Inadvertent injection of bacitracin ointment into the orbit can cause a postoperative orbital compartment syndrome. 

Acute proptosis, chemosis, reduced vision, and ophthalmoplegia occurred after endoscopic sinus surgery in a 73-year-old woman (3). The orbit was tense and the intraocular pressure was 54mmHg. The presence of bacitracin ointment was established by computed tomography. 

Bacitracin is one of the most important clinical allergens (4). Anaphylaxis rarely occurs after topical administration of bacitracin ointment (5,6). 

A 45-year-old man developed a near-fatal anaphylactic reaction after he applied bacitracin ointment to an excoriated area on his foot. He had had a similar, but less severe, episode 4 years earlier. IgE antibodies to bacitracin were positive.

A 3-year-old boy presented with a well-demarcated diamond-shaped skin erosion with bullae. His parents reported the ingestion of several chocolafe squares of the laxative Ex-lax , which he mistook for candies, the night before. The baby was wearing diapers af nighf, which led to overnight contact of his skin with senna-induced diarrhoea. The patient was treated with polymyxin B and bacitracin ointment, with a dramatic improvement in his symptoms within 3 days [67 ].
