Immediate treatment of an exposed individual is essential. The U.S. regimen includes the pretreatment, and after exposure atropine and 2-PAM are self-adininistered. Eurther treatment includes up to two additional doses, followed by the tranquilizer VaHum. As required, artificial respiration is instituted, clearing the airway if necessary. The current standard U.S. Army atropine item is the automatic injector, Atropen, designed for self-adininistration by the individual in the field. 

Children weighing less than 15 lbs (generally younger than 6 months of age) should ordinarily not be treated with the AtroPen auto-injector. Atropine doses for these children should be individualized at doses of 0.05 mg/kg.

Autoinjectors (AtroPen , Mark I , Combopen MC ) are atropine or atropine and pralidoxime combinations available for human use. They are not used in veterinary medicine as they are not adjustable for different sized patients. Experimental vaccines against nerve agent VX, and monoclonal antibodies which protect against soman, sarin, and tabun toxicity have been produced and are being tested (Dunn and Sidell, 1989 Somani et al, 1992). 

These products are provided by Meridian Medical Technologies, which has partnered with the US Department of Defense to be the only FDA-approved supplier of nerve agent antidotes. The Mark I kit and the single autoinjector devices deliver 600 mg of 2-PAM Cl and 2 mg of atropine (AtroPen ) in seconds. This kit was developed originally...

In the late 1940s, atropine was chosen as the standard antidote despite extensive searches for other antidotes since, it has remained the standard. A dose of 2 mg was chosen for self- or buddy-administration (the AtroPen automatic injector included in the MARK I kit contains 2 mg) because it reverses the effects of nerve agents, the associated side effects of a dose this size can be tolerated, and reasonably normal performance can be maintained by the individual receiving it.
