Anti sertraline

Among recent examples that highlight the synthetic utility of transition metal-catalyzed hydroborations are its direction toward a formal syntheses of the non-steroidal anti-inflammatory agents Ibuprofen 131 and Naproxen 13214 15 139 as well as the anti-depressant Sertraline 133 (Figure 14).140 In the majority of cases, rhodium-catalyzed hydroboration is utilized and the rhodium(i) source generally is Wilkinson s catalyst RhCl(PPh3)3. 

Selective serotonin reuptake inhibitors. Currently available selective serotonin reuptake inhibitors (SSRIs) include fluoxetine, paroxetine, sertraline, fluvoxamine, and citalopram. At present, expert opinion does not support the usefulness of these serotonergic compounds in the treatment of core ADHD symptoms (National Institute of Mental Health, 1996). Nevertheless, because of the high rates of comorbidity in ADHD, these compounds are frequently combined with effective anti-ADHD agents (see Combined Pharmacotherapy, below). Since many psychotropics are metabolized by the cytochrome P450 system (Nemeroff et ah, 1996), which in turn can be inhibited by the SSRIs, caution should be exercised when combining agents, such as the TCAs, with SSRIs. 

The molecular ion in the electrospray mass spectrum of the Pfizer anti-depressant Lustral (sertraline hydrochloride), CiyHigNCb, contains the peaks shown in Figure 5.25. Explain the relative intensities of these peaks (given in brackets above each peak). 

It has been known since the mid-1980s that clomipramine, a potent but nonse-lective serotonin reuptake inhibitor, is effective in reducing OCD symptoms. Since then, numerous studies have confirmed the superiority of clomipramine over placebo in OCD patients, whereas other antidepressant medications with less potent inhibitory effects on serotonin reuptake (e.g., nortripytline, desipramine) seem to be ineffective in OCD. Demonstration of the anti-OCD actions of all five SSRIs, namely, fluoxetine, sertraline, paroxetine, fluvoxamine, and citalopram, also supports the hypothesis that the antiobsessional effects of these various pharmacologic agents is due to their potent serotonergic reuptake blocking activity.

When the compound itself contains more than one chiral centre the question of diastereoisomers takes precedence over that of enantiomers. Resolution is normally performed on the wanted diastereoisomer rather than on the mixture. In the case of sertraline 45, an anti-depressant that affects serotonin levels in the brain, the active isomer was not known when both diastereoisomers were prepared by a unselective route.8 The starting material 41 was made by a Friedel-Crafts reaction between 1,2-dichlorobenzene and succinic anhydride. 

In view of these effects the US manufacturers suggest halving the dose of cilostazol in the presence ofCYP3A4 inhibitors such as erythromycin, diltiazem, itraconazole, and ketoconazole. However, the UK manufacturers contraindicate CYP3A4 inhibitors, and they specifically name erythromycin, diltiazem, ketoconazole, cimetidine, and the protease inhibitors. Just why these recommendations differ is not clear. The US manufacturers suggest that other CYP3A4 inhibitors, such as azole anti-fungals (fluconazole, miconazole), SSRIs (fluoxetine, fluvoxamine, sertraline) and nefazodone, may also interact. ...
