Hypersensitivity to any component of the product. The catabolism of lovastatin, simvastatin, and atorvastatin proceeds chiefly through CYP3A4, whereas that of fluvastatin and rosuvastatin is mediated by CYP2C9. Pravastatin is catabolized through other pathways, including sulfation. The 3A4-dependent reductase inhibitors tend to accumulate in plasma in the presence of drugs that inhibit or compete for the 3A4 cytochrome. These include the macrolide antibiotics, cyclosporine, ketoconazole and its congeners, HIVprotease inhibitors, tacrolimus, nefazodone, fibrates, and others (see Chapter 4). Concomitant use of reductase inhibitors with amiodarone or verapamil also causes an increased risk of myopathy.

Severe myopathy has been attributed to the combination of simvastatin with clarithromycin and amiodarone (114). 

A 56-year-old man taking simvastatin was given clarithromycin and amiodarone for pneumonia and a supraventricular tachycardia. He found it difficult to move and complained of general weakness and muscle pain. The blood creatine kinase activity was over 20 000 IU/...

An interaction of simvastatin with amiodarone has been described. 

A 63-year-old white man with insulin-dependent diabetes and recent coronary artery bypass surgery developed diffuse muscle pain with generalized muscular weakness after taking amiodarone 1 g/day for 10 days then 200 mg/ day plus simvastatin 40 mg/day (37). He had a significant increase in creatine kinase activity, peaking at 40 392 U/l.

Reported bioanalytical applications that use SS-LLE products include both tlie cartridge (tube) format and the microplate format. Some of these applications include the determination of mexiletine [44], amiodarone [45] and other antiarrhythmic drugs [46], proxyphylline [47], 16(3-hydroxystanozolol [48], dextromethorphan [49], and simvastatin [50] from biological fluids. Microplate applications for SS-LLE include a crude purification of crude combinatorial library samples [51], carboxylic acid-based matrix metallo-protease inhibitors [52], and a p3-adrenergic receptor agonist [53].
