In 1997 there was an urgent phone call from a chemical works to the ambulance service. There had been an accident in the laboratory at the works. A flask had broken and the solution of phenol it contained had sprayed out onto the forearm of the worker. It was not much, but it was enough. All hospital plans should include provisions for a proportionate response to the arrival of contaminated casualties, with procedures for rapid lock-down and establishing entry and exit restrictions. Most ambulance service plans already provide for sending an immediate alert to all hospitals designated to receive casualties whenever a major incident is declared. In a civil setting, the sudden arrival of a large number of victims at an emergency medical facility or calling the ambulance service may be the first indication of an attack. Early detection of an attack may be based only on presenting toxidromes with specific detection only possible after casualties have presented. Detection systems using infrared spectroscopy are in use with a number of emergency services.

MOAs are required with local MTFs, local emergency medical services, ambulance services, and regional or state emergency medical services officials. The MOAs and frequent coordination with these agencies are necessary to ensure that appropriate off-post resources will be available for support during a chemical accident or incident.

If the situation becomes too large for the ERT to handle then they can call for help from outside organizations, including the local fire department, ambulance services, and other facilities in the area. The emergency plan must take into account the fact that these people are not familiar with the particular process where the emergency has occurred. Where possible, these outside agencies should have the opportunity of training with the plant ERT. 

The local off-site response organizations that are generally available and that perform emergency response functions. These may include police, firefighters and rescue brigades, ambulance services, and control teams for hazardous materials. 

If breathing or heart has stopped, immediately call professional help by dialling 999 or 112 and asking for the ambulance service. Give precise directions to the scene of the accident. The casualty stands the best chance of survival if the emergency services can get a rapid response paramedic team quickly to the scene. They have extensive training and will have specialist equipment with them.

CPR alone does not replace defibrillation in an SCA incident. CPR merely gives a person precious time until medical help arrives. Yet, many ambulance services arrive several minutes after the call. According to the American Heart Association, the chance of survival decreases 10 percent with each passing minute that the heart beat is not returned to normal. Very few people have survived after 10 minutes. Unfortunately, experts can t tell you why SCAs occur nor how to prevent them. They can, however, tell you the best known way to fix them — AEDs.
