Warnings ignored

When a warning is received, many operators are too ready to assume the alarm is out of order. They thus ignore it or send for the instrument mechanic. By the time the mechanic confirms that the alarm is correct, it IS too late. For example ... 

In tlie Jolmstown flood, tliere was no adequate evacuation or safety phui. Once the dam had broken, the water reached tlie city in a matter of minutes - hardly enough time for a city to react. Howeier, tlie warnings of inspectors should liave been taken seriously. Jolmstown, a city of 30,000 people, was destroyed because of the carelessness and ignorance of a few. ... 

Warn boys not to do it - if ignored then call police and/or fire brigade. Remain near scene to give witness information to police. 

Theodore Klumpp, the Food and Drug Administration s chief medical officer, who interviewed Watkins soon after disaster struck, expressed amazement at the chemist s "revelation of ignorance" (22). For anyone alert to the literature, warning signs abounded. 

Have possible warning signals been ignored ... 

The widespread occurrence of penicillin sensitivity, and the survival of penicillin residues in meat following cooking, led Katz to point out that "since up to 10% of the population is potentially sensitive to penicillin and its breakdown products, the risk is too great to be ignored," and to warn against injections unless these are carefully controlled. The use of withdrawal procedures should protect consumers against possible sensitivity reactions from penicillin residues. 

Shoemaker et al. [37] give several examples of the reduction in the number of experimental runs that can occur when it is assumed that some of the terms in the full second-order model are negligible. The reader is warned, however, that assuming a term is negligible is not an assurance that it can be ignored. The presence of terms in the true model that were assumed negligible will bias the estimates of the other coefficients. 

In Britain, the medical community was alerted to the risks by an editorial in the British Medical Journal in 1971, but it was only in 1973 that the Committee on Safety of Medicines advised against the use of diethylstilbestrol during pregnancy (21). In Britain, drugs were commonly not labelled with information about their contents, nor with warnings of risk until well into the 1990s thus, patients were kept in ignorance. No measures have yet been taken in Britain to alert the public to the need for medical surveillance of women who have been exposed to diethylstilbestrol in utero. 

Because the public ignored these warnings and laws, causing a serious health hazard, the FDA had to reissue warnings on GHB in 1997 and 1998, and extended its warnings to include GBL and BD. On January 21, 1999, the FDA asked manufacturers to recall their GBL-... 

Initial Divide-by-0 warning is a byproduct of this step. Ignore it ... 

Select Restraints. .. under the Setup menu. The window shown in Fig. 17.15.3 will appear. The two named torsion angles should appear under the Selections list. Click on each of these names in turn and click on the Add - button, moving the two names to the Restraints list. Ignore the fact that this refers to restraints - that applies to minimization within HyperChem. Be warned that if other named selections appear under either of the Selections or the Restraints lists they will interfere with the refinement process and must be removed. This is done using the Select... option under the Select menu by clicking on each extra name and the Remove button in turn.
