On rare occasions an organic aciduria occurs not because of an enzyme deficiency but from a failure to transport or activate a water-soluble vitamin that serves as a cofactor for the reaction in question. Thus, congenital deficiencies in the metabolism of vitamin B12 commonly give rise to methylmalonic aciduria (Fig. 40-1, Table 40-2). Similarly, deficiencies of biotin metabolism can cause a severe organic aciduria (Table 40-2). It is very important to be aware of the defects of vitamin metabolism because the administration of large doses of these cofactors may completely prevent brain damage. [Pg.669]

The exact mechanism for this dermopathy is unknown, but it has been speculated that kava may interfere with cholesterol metabolism, leading to a reversible, acquired ichthyosis similar to that seen with the use of lipid-lowering agents such as triparanol (3). Skin biopsies of two recent cases associated with use of the commercially available product have revealed lymphocytic attacks on sebaceous glands, with subsequent destruction and necrosis caused by CD8+ cells (see Section 5) (26). Yet another theory involves interference with B vitamin metabolism or action (27). [Pg.33]

In addition to nutritional inadequacy, vitamin deficiency may result from malabsorption, effects of pharmacological agents, and abnormalities of vitamin metabolism or utilization. Thus, in biliary obstruction or pancreatic disease, the fat-soluble vitamins are poorly absorbed despite adequate dietary intake because of steatorrhea. Absorption, transport, activation, and utilization of vitamins require the participation of enzymes or other proteins whose synthesis is under genetic control. Dysfunction or absence of one of these proteins can produce a disease that is clinically indistinguishable from one caused by dietary deficiency.
