Tretinoin 1% has also been used as a chemical peeling agent [22,23]. The efficacy of tretinoin peels was compared with glycolic acid peels in the treatment of melasma in dark skinned patients [23]. In a split face study of ten Indian women, 1% tretinoin was applied to one half of the face, while 70% glycolic acid was applied to the opposite side. Peels were performed weekly. Significant improvement occurred on both sides as assessed by photographs and a Modi-... [Pg.144]

Oral and topical retinoids may cause disruption of the stratum corneum and thereby enhance the depth of penetration of the glycolic acid peel. Therefore, patients on oral isotretinoin or topicals, such as tretinoin, ada-palene, or tazarotene, may experience increased reactivity to the glycolic acid peel. It is then recommended that the topical agents be stopped at least 7 days in advance of the peel. Because isotretinoin needs to be a continuous therapy and there are questions concerning delayed healing with this therapy, it is best left to the physician s expertise when to undertake glycolic acid peels in this setting. [Pg.28]

This can occur unexpectedly if a patient does not inform the physician that they have not discontinued their topical retinoids (Tazarotene, Adapalene, tretinoin), at least 1 week prior to the peel. This occurs because topical retinoids can enhance the penetration of the glycolic acid peel. Another risk of increasing peel penetration is prior treatment of the skin with aggressive scrubbing with loofahs or... [Pg.31]

Following the peel the skin is carefully observed for any complications such as hyperpigmentation and infection. Results are maintained with serial peels and by using at-home tretinoin or glycolic acid, as well as by sun avoidance. [Pg.19]

In the evening, a 0.05% tretinoin cream should be applied (see Chapter 2). This cream reduces the thickness of the stratum corneum and provides a deeper peel with the same concentration. A 10% (non-photosensitizing) glycolic acid gel could be applied on younger women in the morning. [Pg.60]
