The natural trend in drug development will be toward indications of lower incidence, prevalence, and market value. There will be no choice. In addition, attention will have to be increasingly given to individualized medicine. But genomics-based, individualized medicine assumes huge changes in society s attitudes. Both methods presented are based on statistical analyses of a large set of several thousands of proven dmg molecules. In the past few deeades there has been a hiatus in the momentum of research and discovery of novel medicinal compounds . This particular trend in drug development perhaps is augmented due to two vital factors, namely first, strict empirical and rational approach to drug design and secondly, high standards of safety and therapeutic efficacy together with tremendous increased costs of research and development and finally the clinical trials. 

Today it appears that future trends in drug development and industrial pharmacokinetics will be driven by three factors the emergence of genomics, advances in bioanalytical technology, and the need for rapid proof of concept and better surrogate markers for drug efficacy.

Combined with the trend of small niche companies in drug development has come the parallel corporate trend of downsizing and the hesitancy to expand divisions to take on temporary projects. A great number of large companies are contracting for critical aspects of drug discovery and development. Compounds can be manufactured xmder contract. 

As already indicated, this chapter is a broad-brush picture of community services, which it is hoped will be of interest in areas and countries with other types of provision. The book in general concentrates on actual clinical treatment, and the special aspects of treatment in particular patient groups are discussed in the final chapter. Keeping matters very general, there have been three further significant trends in the development of community drug services in the UK.
