Telephone reporting

All telephone points can be considered a method of notification. Telephones can be easily placed in a facility but may be susceptible to ambient noise impacts and the effects of a fire or explosion. Additionally information from verbal sources can be easily misunderstood of spoken during an emergency. Simultaneous use of the phone system during emergency situations may also cause it to be overloaded and connections difficult to achieve.

Time Frames. The time period for submitting written IND safety reports has been revised from ten working days to 15 calendar days. For telephone reports (fatal and life-threatening unexpected reactions), it has been revised from three working days to seven calendar days. Such reports can also be made by fax. Telephone reporting was previously restricted to clinical studies conducted under the IND, but under the new rule, telephone reporting within seven calendar days applies to any unexpected fatal or life-threatening reaction from any source.

In the case of telephone or verbal orders or for telephonic reporting of all types of critical test results, verify with care the entire order or test results by having the person receiving the transmitted information accurately record and "read back" the total order or test results in question. 

Railroads NTSB requires reports of all railroad incidents in which there is a fatality, injuries to five or more people, or more than 150,000 damage. The Federal Railroad Administration requires the same reporting. Railroads must also file a telephone report (with written follow-up) of any signal system failure. Each railroad must report and maintain a log of all incidents arising out of railroad operations. There are three incident classes ... 

Incident Reports During transportation a carrier must report to DOT any unintentional release of a hazardous material. When an incident involves death, serious injury, major property damage or certain releases of radioactive and etiologic agents, the carrier must make an immediate telephone report. DOT regulations detail these reporting procedures.

In Australia, an ADR reporting system for consumers is planned.
