Many surgeons routinely use a patch of autologous vein, or synthetic material, to close the artery, enlarge the lumen and so reduce the risk of restenosis and, more importantly, of stroke. Patching increases the surgery time and there are complications, including rupture... 

Risk factors in surgical site infections include types of surgery (clean, clean-contaminated, contaminated, or dirty), advanced age of patient, obesity, malnutrition, extended hospitalization, diabetes, extended surgery time, immune compromise, and inappropriate antibiotics used prophylactically to prevent postsurgical infections [12]. 

SMP foams have been proposed as a treatment for intracranial aneurysms. An SMP foam would allow for the placement of a single compacted device that would gently expand to fill the entire aneurysm (Fig. 5). This would potentially reduce the complexity of multiple coil placement, avoid coil-induced rupture, decrease surgery time, and allow for a porous matrix for cell invasion and neointima formation to seal the neck of the aneurysm. 

Skeletal muscle is responsible for maintenance of stmctural contours of the body and control of movements. Extreme temperature, sharp traumas or exposure to myotoxic agents are among the reasons of skeletal muscle injury. Tissue engineering is an attractive approach to overcome the problems related to autologous transfer of muscle tissue. It could also be a solution to donor shortage and reduction in surgery time. The studies demonstrate the absence of toxic residuals and satisfactory mechanical properties of the scaffold [41]. 

Nerves are rarely damaged during the total joint replacement surgery. However, nerve damage can occur if considerable joint deformity must be corrected in order to implant the prosthesis. With time these nerves sometimes return to normal function. 

In the veterinary as in the human patient, neoplasms are often metastatic and widely disseminated throughout the body. Surgery and irradiation are limited in use to weU-defined neoplastic areas and, therefore, chemotherapy is becoming more prevalent in the management of the veterinary cancer victim (see Chemotherapeutics, anticancer). Because of the expense and time involved, such management must be restricted to individual animals for which a favorable risk—benefit evaluation can be made and treatment seems appropriate to the practitioner and the owner. In general, treatment must be viewed not as curative, but as palliative. 

The most extensive experience with tissue adhesive drug delivery exists in the fibrin sealant literature. This is clearly an off-label use of this agent as it is only currently approved for hemostasis in cardiac and splenic trauma surgery and for colon sealing at the time of colostomy closure. 

Mr. Davidson, who is 67 years old, is to be discharged after major bowel surgery. The primary health care provider gives him a prescription for 24 tablets of Zolpidem (Ambien). When reading Mr. Davidson s chart you note that he works part time on weekends as a bartender. Discuss what you would emphasize when explaining the prescription to Mr. Davidson.
