Surgery deformity

Nerves are rarely damaged during the total joint replacement surgery. However, nerve damage can occur if considerable joint deformity must be corrected in order to implant the prosthesis. With time these nerves sometimes return to normal function. 

Finite-element techniques can cope with large, highly non-linear deformations, making it possible to model soft tissues such as skin. When relatively large areas of skin are replaced during plastic surgery, there is a problem that excessive distortion of the apphed skin will prevent adequate adhesion. Joint prostheses do not deal with the deformities directly. Orthotics in hemophilia serve as an important supportive measure before or after surgery. 

In a randomized, double-blind study, 14 patients who underwent elective surgery for correction of bilateral arthritic deformities of the feet received 15 ml of 0.9% saline containing diamorphine 2.5 mg into the cannula in one foot and 15 ml of saline into the other foot (6). Intravenous regional diamorphine did not improve postoperative pain relief or secondary hyperalgesia. There were no significant adverse effects.

Therapy is directed at decreasing osteoclastic bone resorption. Bisphosphonates (alendronate, risedronate, pamidronate, and etidronate) and calcitonin are effective in decreasing bone pain, serum ALP, and markers of bone resorption. Patients may occasionally need surgery for skeletal deformity that limits mobility or for arthritic changes, fractures, or nerve compression. 

Referral is always necessary, for orthotic treatment (support or bracing to correct the deformity) or corrective surgery.
