Local irritation Approximately 5% noted irritation in the nose and throat after using sumatriptan nasal spray. The symptoms were transient and, in approximately 60% of the cases, resolved in less than 2 hours.

Treatment of Migraine. Many years ago, ergotamine via metered dose inhaler was used successfully to treat migraine headache. I was made aware of this when I first became interested in medical aerosols by a senior professor of medicine who testified to its efficacy. More recently, sumatriptan nasal spray has been shown to provide significant relief of headaches [132]. 

In another study, 19 healthy subjeets were given a 1-mg dose of butorphanol nasal spray either 1 or 30 minutes following a 20-mg dose of sumatriptan nasal spray. When butorphanol was given 1 minute after sumatriptan the AUC and maximum plasma levels of butorphanol were reduced by about 29% and 38%, respeetively. When butorphanol was given 30 minutes after sumatriptan no signifieant pharmaeokinetic interaction was noted. It was suggested that sumatriptan may eause a transient vasoconstriction of nasal blood vessels, leading to redueed butorphanol absorption. It would therefore seem wise to separate administration to ensure the full effects of butorphanol are aehieved. ...

Administered orally, ergotamine and sumatriptan, eletriptan, naratrip-tan, rizatriptan, and zolmitriptan have only limited bioavailability. Dihydroer-gotamine may be given by i.m. or slow i.v. injection, sumatriptan subcutaneously or by nasal spray.

Sumatriptan was specifically developed as an anti-migraine drug. Injectable sumatriptan has been shown to be more effective than injectable DHE at two hours but the rate of recurrence (30 0%) is higher than with DHE. Efficacy is better and delay to action is shorter with the injectable formulation. Both the nasal spray and the injectable formulation... 

The bioavailability of sumatriptan is only about 15%, but the other agents in the group have availabilities of 40-80%. Sumatriptan, almotriptan, eletriptan, rizatriptan, and zolmitriptan have half-lives of 2-3 hours, while the half-life of naratriptan is 6 hours and that of frovatriptan more than 25 hours. Sumatriptan can be administered subcutaneously by self-injection, as a nasal spray, or orally. The other members of the group are available only for oral administration. 

A 34-year-old man with migraine had palpitation after taking sumatriptan by nasal spray for a severe headache (13). A similar episode had occurred after he had previously taken sumatriptan. He had atrial fibrillation with a rapid ventricular rate. Sinus rhythm returned spontaneously within a few hours. No structural cardiac abnormality was detected. 

Nasal spray sumatriptan can cause taste disturbance, described as a bad, bitter, unpleasant, or unusual taste (20). 

When given snbcntaneonsly, sumatriptan reaches its peak plasma concentration in approximately 12 minutes. Following oral administration, peak plasma concentrations occur within 1 to 2 honrs. Bioavailabihty following the subcutaneous route of administration is approximately 97% after oral administration or nasal spray, bioavailability is only 14 to 17%. The elimination half-life is approximately 1 to 2 hours. Sumatriptan is metabolized predominant-ly by MAO-A, and its metabolites are excreted in the urine. 

No pharmacokinetic interactions or change in adverse effeets were found to oeeur between single 1-mg doses of butorphanol tartrate nasal spray and a 6-mg subeutaneous dose of sumatriptan sueeinate in 24 healthy subjects. It was concluded that concurrent use during aeute migraine attacks need not be avoided. ...

As a consequence of the mucociliary clearance of the nasal cavity active substances that are tmable to pass the nasal membrane will end up in the oropharynx and are swallowed into the gastro-intestinal tract, from where they may be absorbed. This gastro-intestinal absorption after nasal administration may erroneously be considered as nasal absorption. Such a phenomenon may, for example, occur when the nasal spray of the anti-migraine medicine sumatriptan is used.
