We examined ALA-indueed porphyrin fluorescence in various dermatological disorders, particularly skin tumors. In the case of epithelial tumors, correlation was made between the elinieally detectable fluorescence extension and tumor margins as examined histopathologieally [12]. 

The best fluorescence contrast between tumor and normal tissue is obtained if the tumor accumulates porphyrins as much as possible and the normal tissue accumulates very few porphyrins. Fluorescence studies show that superficial lesions of BCC exhibit a maximum porphyrin fluorescence 6 h post ALA application, whereas in nodular lesions the observed intensity is highest after 2-4 h [15]. Detailed biochemical analyses prove that the respective fluorescence intensity correlates well with the extracted quantity of porphyrins [16-19]. In general, surgical excision is the most effective and preferential treatment epithelial skin tumors. However, alternative modalities are necessary for extensive or multiple disseminated lesions such as superficial BCC and solar keratoses to improve functional and cosmetic results. In general, the outcomes of clinical studies on PDT treatment of skin tumors are difficult to compare since different specifications of PDT were used. The following discussion will review mainly data on topical ALA-PDT. The trend of the effectiveness of PDT will be given for different dermatological disorders. Detailed information is summarized in Tables 3-8 below. 

PDT with ALA is very effective in the treatment of SK, small superficial BCCs, and superficial SCCs. Results of PDT in epithelial skin tumors should, however.

Keratins are insoluble proteins that make up such structures as hair, skin, nails, wool, and feathers and form the cytoskeleton structures in all cells of epithelial origin. Along with other fibrous proteins of the same dimensions (e.g., neurofilaments), keratin has been referred to as intermediary filament (IF). In the human being, keratin is one of the more abundant proteins. The basic keratin unit is a relatively small protein with a molecular weight of 40,000-70,000. About 20 different keratin polypeptides have been identified in human hair follicles, various epithelial cells, and tumor cells. They have been assigned numbers 1-20 and have been divided into two classes acidic (type 1) and neutral/base (type 2). A given tissue or cell line will have a characteristic keratin polypeptide distribution, as shown in Table 8.2. However, both types of peptides are always present in a given cell.
