Simvastatin 4- Warfarin

E Itraconazole. Imatinib is primarily metabolized by the CYP3A4 hepatic enzyme system. Drugs that may inhibit this enzyme (such as ketoconazole, itraconazole, erythromycin, clarithromycin, etc.) may impair clearance of imatinib and result in increased toxicity. Imatinib itself is also a fairly potent inhibitor of the CYP3A4 enzyme and may result in toxicity due to other drugs that are substrates for this enzyme (such as simvastatin, warfarin, benzodiazepines, etc.). The other medications iisted are uniikeiy to affect the function of 0 P3A4 or interact adversely with imatinib. 

Clinically important, potentially hazardous interactions with astemizole, atorvastatin, cyclosporine, fluvastatin, lovastatin, pimozide, pravastatin, rifabutin, simvastatin, warfarin... 

The answer is c. (Hardman, pp 887, 889.) Bile acid-binding resins bind more than just bile acids, and binding of simvastatin to cholestyramine is the most likely mechanism for decreased Gl absorption. Cholestyramine may also bind to several other drugs, including digoxin, benzothiadiazides (thiazides), warfarin, vancomycin, thyroxine (T4), and aspirin. Medications should be given one hour before or four hours after cholestyramine. 

Drugs that may be affected by HMG-CoA reductase inhibitors include oral contraceptives, diclofenac, digoxin, glyburide, phenytoin, and warfarin. Atorvastatin, lovastatin, and simvastatin are primarily metabolized by CYP3A4 they may interact with CYP3A4 inhibitors.

In 46 adults taking warfarin, a change from pravastatin to simvastatin caused a significant increase in INR from 2.42 to 2.74 without an overall change in the dose of warfarin there were no unusual episodes of bleeding (92). 

An interaction with warfarin with resulting acute rhabdomyolysis has been observed in a patient taking simvastatin (59). This has not been observed with other statins.

A 61-year-old man, who had taken warfarin and simvastatin for a long time and had an INR of 2.4, started to take cucurbicin five tablets daily for micturition difficulties. After 6 days his INR had increased to 3.4. After withdrawal of cucurbicin the INR returned to the previous value within 1 week.
