System Safety Engineering and Risk Assessment A Practical Approach [Pg.104]

However, the corporate systan safety engineer should chair the group. This section describes the safety representative functions, responsibilities, authorities, how they are part of the SMS, and how they interface and interact across company divisions. It should always be emphasized that safety is everyone s business and responsibility. Many times, company managers or employees will want the safety organization or safety representative to be the responsible party. That should never be the case. The people directly involved with the process and responsible for managing a particular process are always responsible for safety. They cannot hide from this responsibility. [Pg.104]

An important point to note is that companies must allow any worker to stop the production line, without suffering corporate retribution, if someone s life (or bodily injury) is at risk. Workers should not have to wait until the safety engineer notices them and their plight. Many accidents are avoided by the immediate intervention of a coworker during a dangerous situation. This is precisely why we want to analyze to the best of our ability the safety implications of all our processes. Companies that do not do this will surely suffer serious reprimands and fines from the regulator, not to mention possible employee lawsuits. [Pg.104]

Safety representative and personnel Identified individuals in the company responsible for ensuring that the company follows a robust SMS (they are not responsible for safety, which is the responsibility of each and every employee). They also help facilitate efficient safety interface coordination and consultation. [Pg.81]

In the study of the MISHA method, Kuusisto compared his scores on a Finnish company with the scores recorded by the company s safety director, safety manager, safety representative, and personnel manager. He found fair agreement between his scores and those recorded by the safety manager and safety director, and only slight agreement between himself and the other two observers. [Pg.128]

It is hoped that this book will prove valuable to safety advisers, environmental health offieers, emergeney serviees personnel, safety representatives and those engaged in the transport or disposal of wastes - in faet, to anyone involved with ehemieals in the field , i.e. away from ready aeeess... [Pg.618]

Conducting a proper HAZOP study and any supporting what-if analysis is and should always be a team effort. The review team should consist of individuals that work in or are affected by the process under review. These can include supervisors, maintenance personnel, and line employees, as well as safety representatives and even consultants (when necessary) to ensure a well-rounded, experience-based review team. In the end, the team should consist of at least the following types of members ... [Pg.163]

Line managers, SHE personnel (including safety representatives) and decision-makers responsible for design, procurement, training and education, etc. have access to the database for queries. This possibility is utilised infrequently. When used, the majority of the questions are relatively uncomplicated. They mainly concern information on individual accident cases relating to a specific type of machine or job, type of hazard, etc. The database is well suited for the retrieval and summarisation of relevant accident cases. Data on accidents are also used in special studies such as risk analyses and accident concentration analyses. [Pg.341]

If a PSSR is extensive, the PSSR team should include a toller company engineer and an engineer from the client. In addition, an operations supervisory representative and operations personnel with appropriate knowledge and skills along with a maintenance and safety representative may also be included on the team. The PSSR leader must ensure enough manpower and expertise is available on the team to review the new facilities and major modifications thoroughly. [Pg.97]

The personnel responsible for the collection and analysis of incident data vary in different organizations. One common practice is to assign the responsibility to an investigation team which includes the first line supervisor, a safety specialist and a plant worker or staff representative. Depending on the severity of an incident, other management or corporate level investigation teams may become involved. [Pg.266]

For analytical properties to be consistent with the quality expected from an analytical process and the results derived from it, they should be considered in a hierarchical way [4,50]. Thus, there are three primary types of analytical properties, namely (a) capital properties (accuracy and representativeness), which are directly related to quality of the results (b) basic properties (sensitivity, selectivity and precision), which support accuracy and are related to the analytical process and (c) accessory properties (expeditiousness, cost-effectiveness and personnel safety/comfort), which are also related to the properties are related to one another in an additive or contradictory way. The best way of envisaging the ensuing relationships is by means of two analytical tetrahedra sharing a common apex (see Fig. 1.16.A). The apices of the tetrahedron on the left hold the basic analytical properties that define the accuracy triangle, whereas those of the tetrahedron on the right accommodate the accessory analytical properties, which delimit the analytical... [Pg.36]

In the meantime, USA in its mining industry possesses rigorous safety supervision and law enforcement system. The key of its success is represented in up-to-down coalmine safety supervision framework, rotational supervision personnel system, thunder-patterned supervision and law enforcement force. Furthermore, in the structure of the mechanism, a community of interest including the supervisors, mine operators and local governments is avoided. [Pg.694]
