Thus the IMEP programme is a continual learning process for reference laboratories, field laboratories and... [Pg.167]

Some conclusions can be drawn from this discussion. MMS, other federal regulators, and the offshore oil and gas industry failed to develop a cohereut set of safety performance indicators and thereby failed to systematically collect and use iuforma-tiou that could have fueled a continuous learning process, which would iu turn have... [Pg.165]

All the objectives presented in Box 11.1 can be described as quality improvement in the structure and process to support improvement in each patient s health outcome. This support the patient medication care process presented in Fig. 3.1. Various problems and tools and models for improvement have been described in this book. Another approach to prevent medication errors and to improve care is to be open and continuously learn from mistakes. The basis for this is not to punish health care providers who make errors, as this may lead to less reporting of errors. In Britain, the government has taken steps away from this blame-culture (Wise 2001). In a declaration it is stated that honest failure should not be responded to primarily by blame and retribution, but by learning and by a drive to reduce risk for future patients. [Pg.130]

There are several consortia based at universities (CPAC, CPACT) which provide a forum for continuous learning about process analysis and new developments in terms of devices (NeSSI), analyzers (microinstrumentation), and data analysis. These consortia provide a forum for discussing needs, and a means for industries to work together in the development of tools which will support their PAT needs. They can help to bring together experts from numerous industries. [Pg.538]

After World War II, researchers from Dow visited the German polystyrene plants and were surprised to learn of their scale and sophistication. One of the key people on the investigating team that went to Germany was Dr Goggin, founder of Dow s Plastics Technical Service Department. The American teams that visited I. G. Farben after the War recorded their findings in a historic report [7]. This report clearly showed the advantages of a continuous production process for polystyrene. Further, the first industrial production of SAN was in 1936 also by I. G. Farben in Ludwigshafen. [Pg.10]
