A one-page memory-jogging safety assessment checklist was made available by Trevor Kletz in 1976. [6] The checklist is still valid today. After a review of several Management of Change policies from several major companies, it appears that Kletzs checklist or a similar checklist was used as a basis for a few companies procedures. 

Methods for performing hazard analysis and risk assessment include safety review, checklists, Dow Fire and Explosion Index, what-if analysis, hazard and operability analysis (HAZOP), failure modes and effects analysis (FMEA), fault tree analysis, and event tree analysis. Other methods are also available, but those given are used most often. 

A management-risk analysis method would not only inform safety cases, it would also be complimentary to an organisation s audit function. For example, a checklist to inform audits could be made of processes or activities that are deemed particularly risky or that rely on assumptions in which there is limited confidence. In addition, the frequency or thoroughness of audits and the focus of safety assessments may also be increased for parts of a management system that are considered similarly risky, or whose failures could lead to particularly severe consequences. In these ways, both the efficiency and the effectiveness of audits and safety assessments could be improved. 

The lead auditor should prepare a list of questions pertinent to the scope of the audit. ISA-TR84.00.04-1 Annex D.4 Functional Safety Assessment provides example checklists that could be addressed during the audit. In addition, refer to the COPS book, Guidelines for Auditing Process Safety Management Systems," 2011. 

To determine whether it is appropriate to conduct an assessment of safety culture, it is necessary for the investigator to be able to determine the signs that there may be such a problem. It is possible to cross-reference the elements of safety culture to causes in a standard RCA approach, and to turn these cross-references into a checklist for use by the investigator. This provides a direct link for the investigator from the analysis technique with which they are working to safety culture assessment, indicating the areas of safety culture that are likely to require most attention. 

In summary, a successful health and safety policy is likely to lead to a successful organization or company. A checklist for assessing any safety policy has been produced by the HSE and is reproduced in Appendix 2.1.

The essence of the framework and guidelines for safety assessment lies in its way of structuring the assessment procedure and constructiveness of the checklists.
