In addition, responsible parties—from the shop floor to the board of directors—should be rewarded for good process safety performance, and there should be consequences for poor performance. Linking process safety results with personnel compensation must be done carefully to avoid counterproductive incentives. The compensation systems must not discourage reporting unsafe conditions or other gaming of the metrics system. 

The reporting of safety issues must be made easy, simple, and confidential. Many organizations have a telephone number that is a hotline to a recording system. Employees can dial the number in secrecy and report unsafe conditions or acts or other items concerning safety. This gives employees confidence that they can be a part of the safety system without repercussion. Computer terminals can have an input spreadsheet where the issue is reported and submitted to a central receiving office that distributes the item to the correct department or person concerned.

Worksite analysis involves examining the workplace for existing and potential hazards. Comprehensive baseline and periodic safety and health surveys should be conducted. Job hazard analysis, accident, and near-miss investigations should also be held. Workers should be able to report unsafe conditions without fear of reprisal. Trends of illness and injury should be studied over time to identify patterns and prevent problems from recurring.

Report unsafe conditions to your supervisor. If you can safely correct the condition yourself, we encourage you to do so. 

The supervisor records the observation on a specific form. Observations are expected to take 5 to 10 minutes. The form in Figure 20-3 is designed for simplicity. The far right column asks that the employee initial the form to verify that an observation took place. Observations are not intended to be hidden in any manner. The supervisor is expected to alert the employee of this activity. The initialing of the form is also enables the worker to be sure that the reported unsafe conditions are placed on the form.
