Esophagitis reflux

Bisphosphonates Dyspepsia, esophageal reflux, esophageal pain or burning. 

Polymorphisms of CYP2C19 cause differences in metabolism of omeprazole, a proton pump inhibitor used for treatment of gastroduodenal ulcers or reflux esophagitis. Such polymorphisms result in resistance to treatment at a standard dose regimen in nearly 20% of European Caucasians, and in an even higher percentage of Asians [12].

It was reported that healing of reflux esophagitis with 40mg/day of esomeprazole is effective in 78% of patients after 4 weeks of treatment and in 93% of patients after 8 weeks, compared to 65% and 87% of patients treated with 20mg/day of omeprazole.

For prevention of peptic ulcer disease avoiding ulcerogenic medication such as NSAIDs, including aspirin, is probably the most important strategy. Reducing gastric acidity is also the main approach for the treatment of reflux esophagitis.

Contraindications Glaucoma, obstructive uropathy, obstructive disease of Gl-anti-cholinergic tract, paralytic ileus, intestinal atony of the elderly or debilitated patient, unstable cardiovascular status in acute hemorrhage, severe ulcerative colitis especially if complicated by toxic megacolon, myasthenia gravis, hiatal hernia associated with reflux esophagitis, hypersensitivity to any component of f he formulaf ion, acuf e infermiffenf porphyria. 

Anorexia and nausea and vomiting may occur when a dopamine agonist is introduced and can be minimized by taking the medication with meals. Constipation, dyspepsia, and symptoms of reflux esophagitis may also occur. Bleeding from peptic ulceration has been reported. 

Cisapride (27) is effective in the treatment of reflux esophagitis and constipation, with a novel mechanism thought to involve acetylcholine release in the gut. It is devoid of CNS and cardiac side effects [23]... 

In the case of reflux esophagitis, the combination of low-dose NSAIDs coupled to an acid inhibitor (to simultaneously reduce gastrotoxicity and the reflux of gastric juice) may have potent healing properties. The presence of the NSAID may also help to reduce the risk of reflux-related complications such as esophageal carcinoma. 

Methanol poisoning symptoms include vomiting, headache, bradycardia, and hypotension. Presence of formic acid causes retinal damage. Alcohol is contraindicated in peptic ulcers, hyperacidity, reflux esophagitis, epilepsy, liver dysfunction, unstable personalities, and pregnant women. 

In 1988, after extensive documentation and careful evaluation, omeprazole was registered in Sweden for the treatment of duodenal ulcer and reflux esophagitis, and was launched in Europe as Losec in the same year. The clinical program continued to document the value of omeprazole in further acid-related disorders, though its full therapeutic potential became apparent only during the 1990s, when it superseded the Hrreceptor antagonists in the treatment of all these conditions. 

Reflux esophagitis has been reported in patients taking clozapine (179). It has been speculated that reduced esophageal motility was the mechanism (180).
