Like other antihistamines, promethazine has signih-cant anticholinergic effects it should be used with caution in patients with narrow-angle glaucoma, peptic ulcer, or pyloroduodenal obstruction or urinary bladder obstruction from prostatic hypertrophy or narrowing of the bladder neck. It also should be used with caution in patients with CVD or hypertension because of the risk of palpitations with acute or chronic respiratory dysfunction (especially children) as promazine may depress the cough reflex.

Codeine stimnlates opiate receptors in the CNS in addition to cansing respiratory depression, peripheral vasodilation, inhibition of intestinal peristalsis, stimulation of the chemoreceptors that cause vomiting, increased bladder tone, and suppression of cough. They are indicated in the temporary relief of coughs and upper respiratory tract symptoms associated with allergy or the common cold. 

A single report describes a woman taking an MAOI who developed a severe occipital headache after taking 30 mL of a paediatric cough linctus. Initially this interaction was attributed to promethazine, but it is now known that the linctus in question contained phenylpropanolamine, which is much more likely to have been the cause.
