Glucocorticoids should be used with caution in progressive systemic sclerosis, and concomitant administration of anticoagulants to prevent ischemic colitis is recommended when administering glucocorticoids in high doses, especially by pulse therapy (SEDA-21, 415 150).

Scleroderma is a progressive systemic sclerosis with a multisystem connective tissue disease. A number of industrial chemicals have been implicated as causative factors in human scleroderma. Progressive systemic sclerosis is hardly ever complicated directly by stroke, although a carotid and cerebral vasculopathy has been described (Heron et al 1998 Lucivero et al 2004).

Acute renal insufficiency together with diffuse alveolar hemorrhage and bilateral pulmonary infiltrates was suspected to have been caused by penicillamme (500 mg/day for 6 months) in a 34-year-old white woman, who took penicillamine for progressive systemic sclerosis (234). Because of disseminated intravascular coagulation, a biopsy was not made and the role of penicillamine remained uncertain. 

In a long-term prospective study of 69 patients taking penicillamine (750 mg/day) for progressive systemic sclerosis, 27 had adverse effects requiring either temporary reduction or complete withdrawal of therapy. Five of these had two, or, in one case, three different reactions (115).
