Process evolution failures

Companies stumbled in two areas process evolution and technology implementations. These failures drove improvements in supply chain excellence. The pace of change from failure was much faster than from success. 

Failure gave birth to supply chain horizontal processes. Today s leaders in horizontal process evolution—Cisco Systems, Coca-Cola, IBM, Intel, Hewlett-Packard, Samsung, and Walmart—all learned the need for horizontal processes the hard way. Each stubbed its big toe and stumbled before learning that bricks matter. 

Overheating in a vacuum evaporator initiated accelerating polymerisation-decomposition of (endothermic) dicyanobutene, and the rapid gas evolution eventually caused pressure-failure of the process equipment. 

The scenario presented here was developed by R. Gygax [1, 2]. Let us assume that while the reactor is at the reaction temperature (TP), a cooling failure occurs (point 4 in Figure 3.2). The scenario consists of the description of the temperature evolution after the cooling failure. If, at the instant of failure, unconverted material is still present in the reactor, the temperature increases due to the completion of the reaction. This temperature increase depends on the amount of non-reacted material, thus on the process conditions. It reaches a level called the Maximum Temperature of the Synthesis Reaction (MTSR). Although it is believed that the primary defect in early heart failure resides in the excitation-contraction coupling machinery of the heart, the clinical condition also involves many other processes and organs, including the baroreceptor reflex, the sympathetic nervous system, the kidneys, angiotensin II and other peptides, and death of cardiac cells. Recognition of these factors has resulted in evolution of a variety of treatment strategies (Table 13-1). 

The first communication about the Chl-sensitized water oxidation to 02 on the outer vesicle interface conjugated with Fe(CN)g reduction at the inner interface appeared in 1977 [42] (see System 2 in Table 1). Defects in vesicular transport can have the same deadly consequences as the failure to deliver a needed vaccine to a disease-racked city. A related initiative to provide ongoing measurement of the quality of care provided by health systems are the ORYX indicators. ORYX is intended to integrate the use of outcomes and other performance measurement data into the accreditation process. Performance measures are expected to be applicable across the accredited health care organizations with the evolution of core performance measures applicable to each accreditation program. The initial four core measurement areas will be for hospitals and will focus on acute myocardial infarction, heart failure, community-acquired pneumonia, and pregnancy and related conditions. Hospitals are expected to collect data that will allow assessment of patient care based on the standards. The ORYX standards for acute myocardial infarction in Table 21.3 reflect the focus on medications and how pharmacists can impact their organizations performance. ...
