Personnel training inherent safety

Human error is the most frequent cause of the loss of containment. Training and certification of personnel on critical procedures are permanent considerations. Also some other inherent safety principles, like ease of control, making status clear, tolerance and making incorrect assemply impossible, come into play here. 

The standard operating procedures demand great attention as they reflect personnel safety issues, safe operating limits and quality considerations. They should be written simply and clearly. The level of detail is determined by the training and experience of the operations staff but should also take into account the hazards inherent in the process. 

An operating philosophy that trains and rewards personnel for shutting down when required by safety considerations is inherently safer than one that rewards personnel for taking intolerable risks.
