Chapter 3 presents introductory aspects of safety and human factors. Chapter 4 is devoted to methods considered useful to perform patient safety analysis. These methods include failure modes and effect analysis (FMEA), fault tree analysis (FTA), root cause analysis (RCA), hazard and operability analysis (HAZOP), six sigma methodology, preliminary hazard analysis (PFfA), interface safety analysis (ISA), and job safety analysis (JSA). Patient safety basics are presented in Chapter 5. This chapter covers such topics as patient safety goals, causes of patient injuries, patient safety culture, factors contributing to pahent safety culture, safe practices for better health care, and patient safety indicators and their selection. 

The safety culture criteria depicted in Table 9.1 can be understood as the goals or ideal end states of patient safety management and they can be used as a frame to define monitor indicators. Organisational activities can be viewed from the point of view of how they contribnte to these six ideal states. The control functions in Table 9.2 on the other hand can be used for selecting drive indicators that facilitate change in the right direction. 

Thus, they established procedures for clinical trials and other experimentation with humans in which researchers are expected to follow a precautionary protocol for selecting patients and applying experimental methods and products to them, monitoring their response, and then do full and timely reporting of any adverse outcomes to enable open discussion and evaluation of the causes, quick determination of corrective actions needed for patient safety, and rapid dissemination and implementation of the corrective actions in all related research projects. It now appears that this model for safeguarding human subjects needs reinforcement for it to be sufficient in the competitive and economically opportunistic culture in which gene therapies are tested.
