Operational risk management content

Evaluate the Operational Risk Management System in place and consider its adequacy in relation to A Systemic Model for an Operational Risk Management System as shown in Chapter 8. This model should also be considered with respect to the content of a Scoreboard/Dashboard. 

The point is that the innovations chosen to reduce serious injuries and fatalities should pertain specifically to operational needs. While generalities can be suggested with respect to the content and order of elements in an occupational risk management system, emphasis in the application of those elements should result from studies that determine where the largest needs and opportunities lie and where the emphasis should be placed. 

The core function of QRA is to provide information for decision making. QRA results in and of themselves cannot prove anything. However, decision makers can compare QRA risk estimates to their own risk tolerance criteria to decide whether a plant or operation is safe enough. The same QRA results can support both the plant manager s contention that the plant is safe, as well as the community activist s claim that the plant is unsafe. The difference lies in the individual s risk tolerance, not the QRA. 

PESTIDOC has been designed and created as a quick and easy retrieval of documentation on pesticides. The relational database allows the user to find information on the various issues on pesticides and commercial products, to get data needed for the human and environmental risk assessment. So, the database could provide operational guidance and problem solving in case of response and recovery from environmental, human contamination episodes. Furthermore the flexibility of the software allows the implementation of country-specific information and knowledge, such as pesticide commercial names, national MRLs, producers and so on. So the information currently spread in different data repositories could be unified under one ontology, managed by proper content management workflows and published for user-friendly query purposes. 

His first 16 recommendations concerned the requirement for, and content of, safety cases that were to be presented by operators to the safety regulator. The safety cases must address the Safety Management System, the potential major hazards at the platform and the risk to personnel, and emergency evacuation, escape and rescue. 

An RSC is a series of self-prepared documents which are required to contain a number of items. The required contents are listed in table 20.1. One will immediately note the similarities with the safety audits for trucking firms in that RSCs require operators to demonstrate that they have the safety management structure in place. One interesting difference is item number 6 which requires reporting of the significant findings of a risk-assessment exercise. Actually, this requirement dates from 1992 and applies to all British companies who employ more than five people. In general, this item has been found to occupied about a fifth of the total submitted RSC documentation.
