The main indications for atypical antipsychotics are the acute and maintenance treatment of schizophrenic disorders, with an emphasis on the treatment of refractory and chronic disorders. However, because of the lower risk of EPS and in particular of tardive dyskinesia, there is a tendency toward a wider range of indications for some of the atypical neuroleptics. Favorable effects in drug-induced psychoses have been demonstrated for olanzapine. Clozapine seems effective in the treatment and relapse prevention of manic episodes and bipolar disorders, and risperidone has been shown to have good efficacy in conduct disorders and in the pervasive developmental disorders. 

In 2007 Zarrouf and Bhanot published the most extensive recent review and identified 88 reports of NMS associated with six atypical neuroleptics olanzapine, clozapine, risperidone, ziprasidone, quetiapine, and aripiprazole. As a warning to those doctors who cavalierly resume neuroleptics once the NMS has gone into remission, 20 cases showed a clear history of prior NMS, indicating that a patient s first case of NMS predisposes toward another when reexposed to neuroleptics. Olanzapine (Zyprexa) has been touted as being relatively free of the risk of NMS, but the authors located 36 cases.

Casamenti et al. [1399] developed a method for screening 11 central nervous system drugs (phenobarbital, olanzapine, clozapine, risperidone, loxapine, haloperidol, imipramine, amitriptyline, fluoxetine, chlorpromazine, paroxetine) on a Cjg column (A = 230 nm) using a 20/11.7 water (0.4g tetramethylammonium perchlorate with 0.2 mL of 7% (m/m) HCIO4 to pH 2.8 with ammonia)/acetonitrile mobile phase. Keep in mind that perchlorates, when concentrated with some metals, are hazardous. Elution was complete in 35 min with good resolution for most compounds. Plots of the effects of mobile phase modifier level and percent acetonitrile on overall retention are presented. Linear ranges of 25-5000 ng/mL with detection limits of 10-250 ng/mL (analyte dependent) are reported.

A search of Medline, EMBASE and PsycLIT was conducted in August 2000, using the following terms Amisulpride, clozapine, olanzapine, risperidone, sertindole, zotepine, ziprasidone, economics, healthcare, costs. All manufacturers of atypical antipsychotic drugs were contacted in April 2000 and asked to supply primary reference data on their product, and all companies had complied with this request by August 2000. A further manual search was conducted of files and journals kept in the National Centre for Information on Psychotropics at the Maudsley Hospital. Reference sections from all retrieved papers were scrutinized for further relevant references. 

Pharmacoeconomic studies of other dmgs are fewer and, as with clozapine, rely on less robusr merhods. Risperidone is probably associared wirh lower costs. A Swedish study applying UK costs to patient outcomes showed a reduction in mean direct costs of about 7500 per patient per year (Guest et al, 1996). Other studies, however, show risperidone ro be cost-neutral (Revicki, 1999). There are fewer studies of olanzapine (although there are many publications), and taken together they suggest the dmg is at least cost-neutral with respect to immediate costs (Fichner et al, 1998 Hamilton et al, 1999).

Similar interactions as clozapine may need to increase the usual olanzapine dose when a patient begins to smoke, or decrease olanzapine dose if smoking is stopped or nicotine replacement is used instead of smoking.

Pharmacotherapy is the cornerstone of acute and maintenance treatment of bipolar disorder. Mood-stabilizing drugs are the usual first-choice treatments and include lithium, divalproex, carbamazepine, and lamotrigine. Atypical antipsychotics other than clozapine are also approved for treatment of acute mania. Lithium, lamotrigine, olanzapine, and aripiprazole are approved for maintenance therapy. Drugs used with less research support and without Food and Drug Administration (FDA) approval include topiramate and oxcarbazepine. Benzodiazepines are used adjunctively for mania.
